2型糖尿病周围神经病变患者振动感觉阈值和尿微量白蛋白肌酐比值的相关性研究

摘　　要：目的:探讨2型糖尿病周围神经病变(DPN)患者振动感觉阈值(VPT)与尿微量白蛋白之间的关系。方法:选取2014年8月-2015年6月在笔者所在医院确诊的T2DM患者240例,根据症状、体征和实验室检查将其分为糖尿病周围神经病变组(DPN组)和非神经病变组(NDPN组),每组120例。测定两组患者UACR、Cys-c-e GFR,Cys-c、VPT并比较,采用相关分析分析VPT与UACR、Cys-c-e GFR、Cys-c等指标的关系。结果:DPN组的VPT、UACR、Cys-c均明显高于NDPN组,但Cys-c-e GFR明显低于NDPN组,差异均有统计学意义(P<0.05或P<0.01)。相关性分析显示,T2DM患者VPT与UACR和Cys-c成正相关,与Cys-c-e GFR成负相关(P<0.01)。结论:DPN患者VPT水平与UACR密切关系,早期测定T2DM患者VPT水平并采取措施干预,有利于及早防治或延缓DN的发生和发展。Objective：To investigate the relationship between vibrating perception threshold（VPT） and urine microalbuminuria-to-creatinine ratio（UACR） in patients with type 2 diabetic peripheral neuropathy（DPN）.Method：From August 2014 to June 2015,240 patients with type 2 diabetes mellitus（T2DM） in our hospital were selected and divided into the DPN group and the NDPN group according to the symptoms,signs and results of lab examination,120 cases in each group.The UACR,Cys-c,Cys-c-e GFR and VPT between the two groups were detected and compared.The relationship between vibrating perception threshold（VPT） and UACR,Cys-c-e GFR,Cys-c were analyzed by correlation and multiple regression analysis method.Result：Compared with those of NDPN group,the VPT,UACR and Cys-c in the DPN group were significantly increased,and Cys-c-e GFR was significantly decreased,the differences were statistically significant（P〈0.05 or P〈0.01）.Correlation analysis showed that VPT was positively correlated with UACR and Cystatin-C（Cys-c）,and negatively correlated with e GFR based on Cys-c（Cys-c-e GFR）（P〈0.01）.Conclusion：VPT levels in DPN patients are closely correlated with UACR.Early measurement and intervention of VPT level can prevent or delay the occurrence and development of DN in T2 DM patients.
