超声造影联合弹性成像技术在乳腺疾病诊断上的应用价值

摘　　要：目的:评估超声造影联合弹性成像技术在乳腺疾病诊断中的应用价值。方法:对2013年1月至2015年6月在南通大学附属医院行乳腺肿块切除术的120例患者(120个肿块)行术前超声造影和超声弹性成像联合检查,分析联合检查对乳腺病灶诊断的准确率,特异性和灵敏度,并与超声造影和超声弹性成像进行对比。结果:手术切除肿块120个,其中良性肿块83个,恶性肿块37个。超声造影诊断乳腺肿块的准确度,特异性和灵敏度分别为83.3%、83.1%和83.3%,弹性成像技术诊断乳腺肿块的准确度,特异性和灵敏度分别为81.7%、82.1%和81.2%,两种检查手段无统计学意义。超声造影和弹性成像联合应用诊断乳腺肿块的准确度,特异性和灵敏度为95%、90.6%和93.3%,均明显高于超声造影和超声弹性成像,其差异均具有统计学意义(P<0.05)。结论:超声造影和超声弹性成像的单独应用均可用于乳腺肿块的鉴别诊断,然而,超声造影联合超声弹性成像有利于提高乳腺疾病诊断的准确度,特异性和灵敏度。Objective： To evaluate the combination of contrast-enhanced ultrasonography and ultrasonic elastography in the diagnosis of breast diseases. Methods： 120 patients with 120 breast lesions were enrolled from January 2013 to June 2015 in our hospital.All the patients received the combined examination of contrast-enhanced ultrasonography and ultrasonic elastography before surgery. The accuracy,specificity and sensitivity of combined examination was analyzed and compared with either method. Results： 83 benign lesions and 37 malignant lesions were confirmed by pathology. The accuracy,specificity and sensitivity by contrast-enhanced ultrasonography and by ultrasonic elastography were（ 83. 3%,83. 1%,83. 3%） and（ 81. 7%,82. 1%,81. 2%） respectively. No significant difference was found between the two methods. While the accuracy,specificity and sensitivity by combined examination of contrast-enhanced ultrasonography and ultrasonic elastography was 95%,90. 6% and 93. 3%,which was significantly higher than either method（ P 〈 0. 05）.Conclusion： Contrast-enhanced ultrasonography and ultrasonic elastography are both effective in the differential diagnosis of breast lesions,and combined examination of contrast-enhanced ultrasonography and ultrasonic elastography will further improve the accuracy,specificity and sensitivity of the diagnosis of breast diseases.
