超声引导下颈部淋巴结细针穿刺活检与组织病理学比较研究

摘　　要：目的:探讨超声引导下颈部淋巴结细针穿刺活检在诊断颈部淋巴结病变中的应用价值。方法:选择62例颈部淋巴结病变患者为研究对象,均给予超声引导下颈部淋巴结细胞穿刺活检,以术后病理检查为"金标准",分析其诊断效能。结果:62例患者中,穿刺成功57例(57/62);62例患者穿刺组织学诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为90.48%、95.00%、91.94%、97.44%、82.61%;57例穿刺成功患者穿刺组织学诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值分别94.87%、94.44%、94.74%、97.37%、89.47%。结论:超声引导下颈部淋巴结细针穿刺活检诊断效能值得肯定,与手术病理学诊断具有高度一致性,可作为颈部淋巴结病变组织病理学首选检查方法。Objective： To study the application value of ultrasound-guided fine needle biopsy in the diagnosis of cervical lymph lesions. Methods： 62 patients with cervical lymph node lesions were given ultrasound-guided fine needle biopsy,using postoperative pathologic examination as the＂gold standard＂,and analyzed diagnostic performance. Results： 62 patients,puncture successful 57 cases（ 57 /62）; 62 cases patients fine needle biopsy sensitivity,specific,accuracy,positive predictive value,negative predictive value were90. 48%,95. 00%,91. 94%,97. 44%,82. 61%; 57 patients with puncture successful fine needle biopsy sensitivity,specific,accuracy,positive predictive value,negative predictive value were 94. 87%,94. 44%,94. 74%,97. 37%,94. 74%. Conclusion： Ultrasoundguided cervical lymph fine needle biopsy diagnosis effectiveness is worth affirmation,having high consistency with surgical pathology diagnosis,and can be used for cervical lymphadenopathy preferred histopathological examination.
