摘  要:目的检测COPD合并胸腔积液患者血浆及胸水NT-pro BNP水平及其在鉴别心源性和非心源性胸腔积液的意义。方法选择2011年6月~2013年12月入住我院的COPD合并胸腔积液患者80例[其中心源性胸腔积液40例(A组),非心源性胸腔积液40例(B组)],同期在我院住院的无心肺基础疾病的胸腔积液患者40例(对照组)。取胸腔积液行生化、细菌学、细胞学检查及NT-pro BNP水平测定,同时测定血浆NT-pro BNP水平。结果1血浆NT-pro BNP比较:A组〉B组〉对照组,三组间两两比较有统计学意义;2胸水NT-pro BNP比较:A组较B组及对照组均升高,有统计学意义,而B组与对照组比较差异无统计学意义;3以胸腔积液NT-pro BNP〉1 500pg/m L为截点,其诊断COPD合并心源性胸腔积液的敏感性和特异性及约登指数分别为95.2%,86.3%,84.6%。结论COPD并心源性胸腔积液患者胸水及血浆NT-pro BNP均明显升高,对其进行测定有助于COPD并心源性胸腔积液的诊断。Objective To test plasma and pleural effusion NT- pro BNP levels in patients with COPD complicated with pleural effusion to diagnosis the cardiogenic and non cardiogenic pleural effusion. Methods 2011 June 2013 December a choice in our hospital 80 cases of COPD patients with pleural effusion( the cardiac pleural effusion in 40 cases( group A),40 patients with non cardiac pleural effusion( B group)),over the same period in our hospital 40 cases without lung disease in patients with pleural effusion( control group). The pleural effusion for biochemistry,bacteriology,cytology and NT pro BNP levels were measured,and the measurement of plasma NT and pro BNP levels. Results 1. Comparison of plasma NT-pro BNP: A group 〉 B group 〉 control group,three between the two groups was statistically significant. 2. Comparison of pleural NT- pro BNP: A group than in B group and control group were higher. There was statistical significance,but no significant difference between B group and control group. 3. By pleural effusion NT- pro BNP 〉 1500 pg / ml as the cut- off point,the diagnosis of cardiac pleural effusion of sensitivity and specificity and Youden index respectively: 95. 2%,86. 3%,84. 6%. Conclusion In COPD and cardiac pleural effusion in patients with pleural effusion,serum NT- pro BNP increased significantly,and the assay is helpful to the diagnosis of COPD and cardiac pleural effusion.
