2012～2014年菏泽市疑似预防接种异常反应监测分析

摘　　要：目的分析菏泽市2012~2014年疑似预防接种异常反应（adverse events following immunization,AEFI）的发生特征,评价AEFI监测系统运转情况和预防接种的安全性。方法收集AEFI信息管理系统报告的菏泽市2012~2014年AEFI的个案报告,采用描述性方法对相关指标进行流行病学分析。结果菏泽市2012~2014年共计报告AEFI 2 828例,其中一般反应2 767例（97.84%）,异常反应50例（1.77%）,偶合症9例（0.32%）,心因性反应2例（0.07）;男性1 762例,女性1 066例,性别比为1.65∶1,〈2岁共2 261例（79.95%）;〈1岁儿童1 632例,发生数较多的依次为3月龄（279例、17.09%）,5月龄（264例、16.18%）,4月龄（243例、14.89%）和6月龄（214例、13.11%）;多在接种1d内发生（2 416例、85.43%）;发生最多的临床症状为发热/红肿/硬结（1 615例、57.11%）;发生数较多的疫苗依次为百白破（无细胞）疫苗（1 418例）,乙脑（减毒）活疫苗（192例）,流脑A群疫苗（155例）和乙肝（酵母）疫苗（142例）。结论菏泽市AEFI监测的完整性和敏感性逐年提高,需要进一步加强AEFI监测工作,规范预防接种操作,严格掌握接种禁忌证,减少AEFI发生。Objective To analyze the adverse events following immunization（AEFI）reported in Heze city,2012-2014 and evaluate the safety and performance of AEFI surveillance system. Methods The information was collected from the management system for AEFI reported in Heze city,2012-2014,the descriptive epidemiological method was used for analysis on the relevant indicators. Results A total of 2 828 cases AEFI were reported in Heze city,2012-2014,2 767 cases of general reaction（97.84%）,50 cases of abnormal reaction（1.77%）,9coincident cases（0.32%）,2cases of psychogenic reaction（0.07）;2 261 cases of children under 2years old;1 632 cases of children under 1years old,and more cases were in 3months（279cases,17.1%）,5months（264cases,16.18%）,4months（243cases,14.89%）and 6months（214cases,13.11%）;most cases occurred in 1dafter vaccination（2 416 cases,85.43%）;most clinical symptoms were fever/swollen/induration（1 615 cases,57.11%）;more number of vaccines followed by diphtheria-pertussis（acellular）vaccine（1 418cases）,JE（attenuated）vaccines（192cases）,meningococcal serogroup A vaccine（155cases）and hepatitis B（yeast）vaccine（142cases）. Conclusion The integrity and sensitivity of AEFI monitoring increases in Heze city year by year,it is needed to further strengthen AEFI monitoring,standardize the vaccination operation,strictly control vaccination contraindications,so as to reduce the occurrence of AEFI.
