摘  要:目的:探讨电针联合膀胱功能训练对脊髓损伤后神经源性膀胱排尿功能的影响。方法:将76例脊髓损伤后神经源性膀胱患者随机分为对照组和治疗组各38例,两组患者均接受脊髓损伤恢复期的康复科常规治疗和康复训练,并对各类合并症给予对症处理,针对神经源性膀胱,对照组接受训练指导、间歇导尿以及膀胱功能训练,治疗组患者同时予电针治疗,4周后比较疗效。结果:治疗后两组患者的残余尿量均明显下降,而最大膀胱测压容积和最大尿流速率均有明显上升,与治疗前比较,差异均有统计学意义(P<0.05);治疗后两组比较,治疗组残余尿量低于对照组,最大膀胱测压容积和最大尿流速率均高于对照组,差异均有统计学意义(P<0.05)。治疗后两组患者的日间歇导尿量均明显下降,差异有统计学意义(P<0.05);治疗后两组比较,治疗组日间歇导尿量、首次排尿时间和建立反射性排尿时间均低于对照组,差异均有统计学意义(P<0.05)。结论:在常规康复训练的基础上,电针联合膀胱功能训练能进一步改善患者的膀胱排尿功能,尽快建立反射性排尿。Objective: To investigate the effects of electro-acupuncture combined with bladder function training on the function of neurogenic bladder after spinal cord injury. Methods: The 76 cases of neurogenic bladder patients were randomly divided into control group and treatment group, each group have 38 cases, all patients received routine rehabilitation training and treatment, then the intermittent catheterization and bladder function training were given to control group, patients in the treatment group received electro-acupuncture treat- ment, the course of treatment was 4 weeks. Results: The residual urine volume of the two groups were significantly decreased, and the maximum bladder pressure capacity and maximum urine flow rate were significantly increased, compared with the treatment before, the difference was statistically significant (P〈0.05), the same situation exists between the two groups after treatment (P〈0.05).The day of two groups of patients with intermittent catheterization were significantly decreased, there were statistical significance (P〈0.05) compared with before treatment. In the treatment group, the daytime intermittent catheterization, for the first time micturition time and establish reflex micturition time were lower than those of control group (P〈0.05). Conclusion: The electro-acupuncture combined with bladder function training can further improve the patient's urinary bladder function, as soon as possible to establish a reflex micturition time.
