宜昌市夷陵地区40岁以上人群糖尿病与糖尿病前期的现患率及其危险因素

摘　　要：目的:调查宜昌市夷陵地区40岁以上人群糖尿病(DM)与糖尿病前期的现患率及其危险因素。方法:对宜昌市夷陵地区40岁以上人群进行DM与DM前期的流行病学研究。结果:10000名被调查者中DM前期与DM的现患率分别是28.5%、9.1%,女性现患率明显高于男性(P<0.05)。多因素logistic回归分析显示校正了年龄与性别后,DM前期的独立危险因素为收缩压、TC、空腹胰岛素,而DM的独立危险因素为DM家族史、收缩压、TG、空腹胰岛素、ALT、GGT;小学以上教育程度则是DM与DM前期的共同保护因素(P<0.05)。结论:宜昌市夷陵地区40岁以上居民的糖尿病及糖尿病前期流行状况非常严重,应加强普及糖尿病知识,纠正可控制的危险因素,预防糖尿病的发生发展。Objective: To explore the prevalence and risk factors of diabetes( DM) and pre-diabetes in adults aged over 40 years old in Yiling area of Yichang City. Methods: A epidemiological survey of DM and Pre-DM had been carried out in Yiling area of Yichang City for 10000 persons aged over 40 years old. Results: The prevalence of pre-diabetes and diabetes among the 10000 subjects were 28. 5% and 9. 1% respectively,with higher prevalence in female than men( P < 0. 05). Multivariate logistic regression analysis showed that,after adjusted for age and sex,the independent risk factors of pre-diabetes were systolic blood pressure,TC and fasting insulin,while those factors of diabetes were family history of diabetes,systolic blood pressure,TG,fasting insulin,ALT,and GGT; high education level was an independent protective factor for both the pre-diabetes and diabetes( P<0. 05). Conclusion: The prevalence of pre-diabetes and diabetes is severe in residents over the age of 40 in Yichang city in the current stage. More attention should be paid to the related risk factors to prevent the development of diabetes and pre-diabetes.
