凉血祛瘀法干预踝部骨折术后肿胀及关节功能临床观察

摘　　要：目的:探讨凉血祛瘀法干预踝部骨折术后肿胀及关节功能的临床疗效。方法:将300例踝部骨折手术患者随机分为治疗组148例和对照组152例,治疗组术后予以凉血祛瘀方煎汤口服2周,并以舒筋洗外用颗粒外洗4周。对照组术后予以桃红四物汤煎汤口服2周,予以海桐皮汤煎汤外洗4周。于术后第2、4、7、14天及第4、12、20周随访患者,根据视觉模拟疼痛评分(VAS)、肿胀值、踝后足评分系统量表(AOFAS)评分结果分析比较两组疗效。结果:术后第4、7、14天及第4、12、20周随访日,两组VAS评分有显著统计学差异(P<0.01);术后第4、7、14天及第4、12周随访日,两组肿胀值有显著统计学差异(P<0.01);术后第7、14天及第4、12、20周,两组AOFAS评分有显著统计学差异(P<0.01)。结论:相对于传统活血祛瘀法,凉血祛瘀法治疗踝部骨折术后肿胀及关节功能疗效确切,值得推广。Objective：To explore the therapeutic intervention effect of cooling blood expelling bloodstasis on swelling and function of joint in ankle fracture after operation. Methods：Three-hundreds cases were randomly divided into a sticking group of 148 cases and a control group of 152 cases,the sticking group was treated with drinking prescription of cooling blood expelling blood-stasis for two weeks,and rubbing the Shujin Washing Granule for four weeks,while the control group was treated with drinking Taohong Siwu Decoction for two weeks,and rubbing the Cortex Erythrinae Decoction for four weeks in tablets.Observe the swelling,VAS and AOFAS score after treatment,compare with the therapeutic effect between two groups.Results：There is statistics significance between treatment group and control group in the VAS score before and after treatment in 4,7,14 days and 4,12,20 weeks（P〈0.01）. There is statistics significance between treatment group and control group in the swelling score before and after treatment in 4,7,14 days and 4,12 weeks（P〈0.01）.There is statistics significance between treatment group and control group in the AOFAS score before and after treatment in 7,14 days and 4,12,20 weeks（P〈0.01）. Conclusion：Compare with the troditional treatment,the cooling blood expelling blood-stasis is better for the swelling qnd function of joint in ankle fracture after operation.
