炎性因子HSP27和CRP在房颤患者应激反应中的临床意义

摘　　要：目的：探究炎性因子热休克蛋白27（HSP27）及C反应蛋白（CRP）在不同年龄组心房颤动（房颤）患者应激反应中血清的表达及其临床意义。方法：收集符合西医诊断标准的房颤患者60例,按不同年龄段分为A组（60～69岁）20例,B组（70～79岁）18例,C组（80～89岁）22例,正常对照组21例。取房颤患者发作当天静脉血4 m L,采用酶联免疫法（ELISA法）分别测定CRP及HSP27浓度,比较各组间血清CRP、HSP27水平差异。结果：A、B、C三组HSP27水平较对照组均显著升高（P〈0.01）,B组与C、A两组HSP27水平比较差异不明显（P〉0.05）,而C组HSP27浓度高于A组（P〈0.01）。A组及C组CRP水平较对照组显著升高（P〈0.01）,而B组CRP浓度较对照组有升高趋势,但无统计学意义（P=0.078）,C组较A、B两组CRP水平升高明显（P〈0.01）,A、B两组比较无明显差异（P=0.261）。结论：房颤的发生可能与炎症有关,炎性因子水平越高,房颤风险越高。HSP27及CRP水平对房颤的发生和维持起着重要的作用,其中HSP27的表达意义尤其突出,可以作为预测房颤发生的指标。Objective：To study the expression and clinical significance of heat shock protein 27（HSP27）and C-reactive protein（CRP）in stress response of patients with atrial fibrillation（AF）for different ages. Methods：Set in line with the western medicine diagnostic criteria of 60 patients with atrial fibrillation,according to the different age groups were divided into group A（60-69 years）20 cases,18 cases group B（70-79）,group C（80-89）22 cases,21 cases of normal control group. In patients with atrial fibrillation attack on the day of the venous blood 4 ml,using enzyme-linked immunoassay（ELISA）determination of CRP and HSP27 concentration respectively,and compare the serum levels of CRP and HSP27 differences between groups. Results：A,B,C three groups of HSP27 level than the control group were significantly higher（P〈0.01）,group B and C,A HSP27 level is similar between the two groups is not obvious（P st13 0.05）,HSP27 concentration higher than that of group A and group C（P〈0.01）. Group A and group C CRP levels higher than the control group significantly（P〈0.01）,while group B trend of CRP were elevated compared with control group,but no statistical significance（P=0.078）,compared with A,B two group C were significantly elevated CRP levels（P〈0.01）,A,B two groups was no significant difference（P=0.261）. Conclusion：The occurrence of atrial fibrillation may be associated with inflammation,inflammatory factors level is higher,the higher the risk of atrial fibrillation. HSP27 and CRP levels on the occurrence of atrial fibrillation and maintain plays an important role,especially in which the expression of HSP27 significance,can be used as a prediction index of af.
