曲马多与右美托咪定联合中药预防全身麻醉术后寒战的应用研究

摘　　要：目的:探讨曲马多、右美托咪定以及二者分别联合中药预防全身麻醉术后寒战的临床疗效。方法:选择2011年2月—2015年2月于我院就诊并接受全身麻醉术的患者124例,严格按照随机化的原则将所有入选患者分为曲马多组(A组)、右美托咪定组(B组)、曲马多+参附注射液组(C组)、右美托咪定+参附注射液组(D组),每组各31例,观察各组术后的寒战情况和不良反应的发生。结果:卡方分析结果显示,四组患者的呼吸恢复时间、拔管时间以及唤醒时间差异无统计学意义(P恢=2.54,P拔=2.19,P唤=1.94,均P>0.05);经秩和检验,四组预防寒战的效果差异具有统计学意义(P<0.05),疗效从高到低的顺序为:D>B>C>A;四组用药后恶心、呕吐等不良反应的发生比较差异有统计学意义(P<0.05)。结论:与曲马多相比,右美托咪定预防寒战的效果更好,不影响心血管功能,不引起胃肠道反应,临床安全性好。右美托咪定联合参附注射液的效果优于单独使用右美托咪定或者曲马多的疗效。Objective：To investigate the application of tramadol with dexmedetomidine given joint preventive medicine for general anesthesia postoperative shivering patients. Methods：124 cases of patients receiving general anesthesia were selected from February 2011 to February 2015 in our hospital,all patients will be divided into tramadol group（A group）,right Meituo Microphone set（group B）,tramadol ＋ Shenfu injection group（group C）,dexmedetomidine set ＋ Shenfu injection group（group D）in strict accordance with the principle of randomization,each group had 31 cases,and observed the occurrence of chills and adverse events in each group after surgery. Results：Chi-square analysis showed that four groups of patients with breathing recovery time,extubation time and wake time difference was not statistically significant（Precovery = 2.54,Ppull = 2.19,Pcall = 1.94,P〉0.05）.The rank sum test tell,four groups have statistically different effects to prevent shivering（P〈0.05）,the efficacy descending order：D〉B〉C〉A;and the adverse reaction of drug like malignant,vomiting was statistically different（P〈0.05）. Conclusion：Compared with tramadol,dexmedetomidine is better for shivering prevention,it has fewer adverse reactions. Dexmedetomidine given joint Shenfu injection are better than dexmedetomidine alone or tramadol given effect.
