全髋置换术治疗高原地区陈旧性髋关节结核继发骨关节炎

摘　　要：目的回顾性分析全髋置换术治疗高原地区成人陈旧性髋关节结核继发骨关节炎的临床疗效。方法对2006年5月至2011年8月我科收治的23例（23髋）陈旧性髋关节结核继发骨关节炎成人患者行全髋关节置换术,以术前、术后随访时Harris评分评价疗效并进行统计学分析。其中男21例,女2例;年龄28-68岁,平均41.8岁;左髋18例,右髋5例;患侧下肢短缩3-5 cm,平均（3.53±0.87）cm。假体均选用陶对陶生物型全髋关节,带领解剖股骨柄。结果 23例患者均得到随访,随访时间平均22.5个月（12-36个月）。术前Harris评分（37.50±3.78）分,术后评分（87.43±2.81）分,P〈0.05为差异有统计学意义。结论全髋置换术治疗成人陈旧性髋关节结核继发骨关节炎安全、有效,髋臼重建是手术的关键。Objective To retrospective analyze the clinical effect of total hip arthroplasty for osteoarthrits secondry to old tuberculosis of the hip in adult in plateau area. Methods Twenty-three hips of twenty-three cases underwent total hip arthroplasty for osteoarthrits secondry to old tuberculosis of the hip joint in adults. They were evaluated with Harris score of preoperative and post-operative and analysized by statistics. Results All cases were followed up for average 22. 5 months（ range 12 -36 months）. The mean preoperative Hip Harris Score（ HHS） was（ 37. 50 ± 3. 78） compared with postoperative HHS（ 87. 43± 2. 81）（ P〈0. 05）. Conclusion Total hip arthroplasty for osteoarthrits secondry to old tuberculosis of the hip joint is safe and effective. The key is acetabular reconstruction.
