经皮肾镜气压弹道联合超声碎石术治疗复杂性肾结石的临床疗效分析

摘　　要：目的 探讨经皮肾镜气压弹道联合超声碎石术治疗复杂性肾结石的临床效果.方法 按照手术方法的不同将复杂性肾结石患者分为研究组和对照组,其中对照组给予开放手术治疗,而研究组则给予经皮肾镜气压弹道联合超声碎石术治疗.结果 研究组患者在手术时间、术后下床时间、住院时间等均较采用开放手术治疗的对照组明显缩短,而研究组的术中出血量则较对照组明显下降（t=21.36、13.65、6.10、11.95,P〈0.05）.研究组患者的结石清除率（81.48%） 明显高于对照组的结石清除率（46.88%）（x2=15.00,P〈0.05）.研究组术后镇痛药物应用率（24.07%）和术后发热率（12.96%）均明显高于对照组（90.63%、39.06%）（x2=54.13、10.09,P〈0.05）.结论 经皮肾镜气压弹道联合超声碎石术治疗复杂性肾结石的临床效果非常明显.Objective To investigate the clinical curative effect analysis of pneumatic lithotripsy under percutaneous nephrolithotomy combinedwith ultrasonic lithotripsy in treatment of complex renal calculus. Methods According to the surgical methods of different complexity renal calculipatients were divided into study group and control group, the control group was treated with operation development, and the research group was givenpneumatic lithotripsy under percutaneous nephrolithotomy combined with ultrasonic lithotripsy. Results Operation time, postoperative bed time,hospitalization time of the study group was significantly shorter than the control group. While the volume of blood loss during operation in the study groupwas decreased significantly than the control group （t=21.36, 13.65, 6.10, 11.95, P 〈 0.05）. The stone clearance rate of the study group （81.48%） wassignificantly higher than that of the control group （46.88%） （x2=15.00, P〈0.05）. The application rate （24.07%） and postoperative fever rate （12.96%） of thestudy group were significantly higher than those of the control group （90.63%, 39.06%） （x2=54.13, 10.09, P〈0.05）. Conclusion Clinical effect of Bultrasound guided standard channel pneumatic lithotripsy ander percutaneous nephrolithotomy combined with ultrasonic lithotripsy is very obvious.
