健康人踝臂指数与血清胆红素的相关性研究

摘  要:目的研究体检人群踝臂指数(ABI)与血清胆红素的关系。方法选择2014年7月在中国中医科学院广安门医院南区的健康体检者281名,询问病史,测量身高、体重、体重指数(BMI)、血压等,检测血常规、生化等指标,应用欧姆龙BP-203RPEⅢ动脉硬化检测仪检测体检人群的踝臂指数(ABI),根据ABI分为<0.9组(Ⅰ组,60名)、0.9<ABI<1.1组(Ⅱ组,72名)、1.1<ABI<1.3组(Ⅲ组,149名),对3组的生化指标进行比较。结果经单因素方差分析检验,Ⅰ组BMI、心率(HR)、总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)与Ⅱ组、Ⅲ组比较差异有统计学意义(P<0.05)。相关性分析:BMI、DBP、TBIL、DBIL与ABI呈正相关性;HR与ABI呈负相关性。线性逐步回归分析:以ABI为应变量,对BMI、HR、TBIL、DBIL进行Logistic逐步回归分析,结果显示:HR、BMI、TBIL进入了最后的回归方程,回归方程为:ABI=1.152-0.002×HR+0.004×BMI+0.003×TBIL。结论 HR、BMI、TBIL是ABI的危险因素。ABI的升高与低水平的血清胆红素有关,血清胆红素水平可早期预测动脉粥样硬化。Objective To study the relationship between ankle-brachial index(ABI)and serum bilirubin in healthy individuals.Methods Two hundreds and eighty-one healthy individuals were enrolled in the medical center of the Southern District of Guang'anmen Hospital of China Academy of Chinese Medical Sciences in July 2014.The medical history,height,weight,body mass index(BMI),blood pressure,blood routine and biochemical indicators were observed.The ankle-brachial index(ABI)of the crowd was tested by Omron BP-203RPEⅢ Arteriosclerosis Arterial Elasticity Detector.According to ABI,the healthy individuals were divided into groupⅠ(n =60),group Ⅱ(n =72),group Ⅲ(n =149).The biochemical indices were compared between the three groups.Results There were statistically significant differences in BMI,heart rate(HR),serum total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL)between group Ⅰ and group Ⅱ or group Ⅲ(P〈0.05).Correlation analysis showed that BMI,DBP,TBIL,DBIL were positively correlated with ABI,and HR was negatively correlated with ABI.ABI grouping as the dependent variable,BMI HR,TBIL,DBIL Logistic regression analysis showed that HR,BMI,TBIL entered the final regression equation.The regression equation was:ABI=1.152-0.002×HR+0.004×BMI+0.003×TBIL.Conclusion HR,BMI and TBIL are the risk factor of ABI.The elevated ABI is related to low levels of serum bilirubin.The serum bilirubin levels may predict early atherosclerosis.
