新鲜下肢骨折术前深静脉血栓形成危险因素分析

摘　　要：目的研究新鲜下肢骨折术前深静脉血栓(DVT)形成的危险因素。方法选取我院124例新鲜下肢骨折患者为研究对象,收集患者年龄、性别、体重、吸烟史、饮酒史、是否存在体内肿瘤、是否长期卧床几项指标的临床资料,通过单因素分析及多因素Logistic回归分析新鲜下肢骨折术前DVT的危险因素。结果本次研究124例患者共18例术前出现DVT,DVT发生率14.52%,单因素分析结果显示年龄、BMI、吸烟、体内肿瘤及长期卧床为术前深静脉血栓形成危险因素,而性别、饮酒史与DVT发生无明显相关性;多因素Logistic回归分析提示饮酒为术前深静脉血栓形成的保护因素,而年龄、BMI、吸烟、体内肿瘤及长期卧床均为新鲜下肢骨折术前深静脉血栓形成的独立危险因素。结论年龄、BMI、吸烟、体内肿瘤及长期卧床是新鲜下肢骨折术前深静脉血栓形成的危险因素,临床需加强对具备以上特征患者的病情监测,以减少术前深静脉血栓出现,保证患者生命安全。Objective To study the risk factors of preoperative deep venous thrombosis（ DVT） in fresh lower limb fractures. Methods 124 patients with lower limb fractures in the hospital were selected as the research objects. The clinical data like patient＇s age,gender,weight,smoking history,drinking history,whether having in-vivo tumor or not and whether long-term in bed or not were collected. Univariate analysis and multiple Logistic regression analysis were used to analyze the risk factors of preoperative DVT in fresh lower limb fractures. Results Among the 124 patients in this study,there were 18 cases with preoperative DVT. The incidence rate of DVT was 14. 52%. The results of univariate analysis showed that age,BMI,smoking,in-vivo tumor and long-term in bed were the risk factors of preoperative DVT while there was no significant correlation among gender,drinking history and the occurrence of DVT. Multiple Logistic regression analysis indicated that drinking was a protective factor for preoperative DVT while age,BMI,smoking,in-vivo tumor and long-term in bed were the independent risk factors of preoperative DVT in fresh lower limb fractures. Conclusion Age,BMI,smoking,in-vivo tumor and long-term in bed are the independent risk factors of preoperative DVT in fresh lower limb fractures. In clinic,it needs to strengthen the monitoring of patients with the above characteristics to reduce the incidence of preoperative DVT and ensure the safety of patients.
