摘  要:目的研究不同年龄段H型高血压合并慢性心力衰竭患者血液中同型半胱氨酸(Hcy)合成关键酶甲基四氢叶酸还原酶(MTHFR)、胱硫醚合成酶(CBS)和蛋氨酸合成酶(MTR)的表达特点。方法 2012年1月-2013年9月云南省第三人民医院心内科收治H型高血压合并慢性心衰患者64例,根据年龄分为三组:〈50岁为B组(7例),50-70岁为C组(44例),〉70岁为D组(13例);另选取30例体检健康人为正常对照组(A组)。采用ELISA测定炎性因子、总抗氧化能力(T-AOC)和同型半胱氨酸(Hcy)含量,Western blot及RT-PCR半定量检测全血中MTHFR、CBS和MTR m RNA及蛋白表达水平。结果 B组、C组及D组患者的血管紧张素Ⅱ(AngⅡ)、内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、丙二醛(MDA)和Hcy明显高于A组,差异有统计学意义(P〈0.05);B组、C组以及D组的T-AOC、超氧化物歧化酶(SOD)、谷胱甘肽(GSH)明显低于A组,差异有统计学意义(P〈0.05);RT-PCR以及Western blot结果显示:B组、C组及D组患者血清中MTHFR、MTR表达明显低于A组,差异有统计学意义(P〈0.05);B组与C组的CBS表达相较A组降低,但差异无统计学意义(P〉0.05),D组CBS表达明显低于A组,差异有统计学意义(P〈0.05),D组MTHFR、MTR、CBS表达均明显低于B组,差异有统计学意义(P〈0.05)。结论 H型高血压合并慢性心衰患者血清中Hcy含量与年龄有关,这可能是Hcy合成相关酶MTHFR及MTR表达水平降低造成。Objective To study the expression of serum Hcy synthesis key enzyme MTHFR, CBS and MTR in H-type hypertension patients with chronic heart failure. Methods From January 2012 to September 2013, in the Third People's Hospital of Yunnan Province, 64 H-type hypertension patients with chronic heart failure were selected, According to the patient's ages, they were divided into three groups, 50 years were as group B(7 cases), 50-70 years were as group C(44 cases), 70 years were as group D(13 cases) over, 30 healthy people in physical examination were selected as group A. The expression of inflammatory cytokines, total antioxidant capacity were detected by Elisa, m RNA and protein of MTHFR, CBS and MTR were detected by Western blot and RT-PCR. Results The levels of Ang Ⅱ, ET-1,TNF-α, IL-1β, MDA and Hcy in group B, group C and group D were significantly higher than those in group A, the differences were statistically significant(P〈0.05); The levels of T-AOC, SOD, GSH in B group, group C and D groups were significantly lower than those in group A, the differences were statistically significant(P〈0.05); MTHFR and MTR m RNA and protein expression in group B, group C and group D were significantly lower than those in group A,the differences were statistically significant(P〈0.05), CBS expression in group D was significantly lower than that in group A, the differences were statistically significant(P〈0.05). Conclusion The level of serum Hcy in H-type hypertension in patients with chronic heart failure is relevant with age, which maybe due to Hcy synthesis enzymes MTHFR and MTR lower expression caused.
