AIHA患者应用两种配血方法的输血疗效比较

摘　　要：目的通过比较自身免疫性溶血性贫血（AIHA）患者分别采用微柱凝集法和体外溶血试验两种配血方法进行输血的疗效，为AIHA患者寻找一种安全有效的配血方法。方法选取北京大学深圳医院2010年1月～2014年12月诊断为AIHA并且需要输血治疗的患者75侧，随机分为A组40例与B组35例，A组采用微柱凝集法配血共输血65人次，B组采用体外溶血试验配血共输血63人次，比较两组患者输血疗效。结果两组患者输血治疗后RBC计数与Hb水平均有上升，TBIL含量均下降。B组患者RBC计数，Hb水平升高幅度及TBIL含量下降幅度明显高于A组患者；两组Hb水平输血前比较差异无统计学意义（P〉0．05），输血后比较差异有统计学意义（P〈0．05），表明B组输血疗效优于A组。A组输注总有效率为67．69％，B组输注总有效率为93．65％，B组总有效率明显高于A组，两组比较差异有统计学意义（P〈0．05）。结论AIHA患者采用体外溶血试验配血方法可以筛选到更适合的供者红细胞，确保患者输血安全有效。Objectives To compare the efficacy of two methods of blood transfusion in the patients with autoimmune hemolytic anemia （AIHA） which was treated by micro column agglutination test and extraeorporal hemolysis test,and find a safe and effective method for AIHA patients. Methods 75 patients who were diagnosed as AIHA in Shenzhen Hospital of Beijing University from January 2010 to December 2014 were treated with blood transfusion. Randomly divided into A group of 40 cases and B group of 35 cases. A group with a total of 65 blood transfusion by micro column agglutination,and the B group with a total of 63 blood transfusion by extracorporal hemolysis test. Then compared the efficacy of blood transfusion in the two groups. Results The RBC counts and Hb levels were increased in the two groups after blood transfusion treatment, and the TBIL content decreased. The RBC count, Hb level and the TBIL level in the B group were significantly higher than those in the A group. There was no statistical significance in two groups of Hb levels before blood transfusion （P〉 0.05） ,and there was statistical significance after blood transfusion （P〈0.05）. The effect of A group was better than that of B group. The total effective rate of A group was 67.69% ,and the total effective rate of B group was 93. 650/00. The total effective rate of B group was significantly higher than that of A group,the difference was statistically significant （P〈0.05）. Conclusion In patients with AIHA,the extracorporal hemolysis test can be used to screen more suitable donor red blood cells, to ensure the safety and effectiveness of patients with blood transfusion.
