摘  要:目的探讨葡萄多酚(GSPE)对糖尿病大鼠膀胱功能的保护作用及其机制。方法将30只大鼠随机分为对照组、DM组、GSPE组各10只。DM组、GSPE组腹腔注射链脲佐菌素制备糖尿病模型,对照组腹腔注射相同剂量的柠檬酸钠缓冲液。造模成功后,GSPE组按250 mg/(kg·d)GSPE灌胃1次/d,对照组、DM组以等量生理盐水灌胃,均持续8周。采用尿流动力学方法,在膀胱容积不变的条件下测定各组膀胱逼尿肌的最大收缩压力及静息压力。处死各组大鼠后取膀胱组织,行HE染色观察膀胱病理组织学表现,采用分光光度法检测膀胱组织中氧化应激指标包括丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平。结果 DM组、GSPE组膀胱逼尿肌最大收缩压力均低于对照组(P均<0.01),GSPE组膀胱逼尿肌最大收缩压力高于DM组(P<0.01)。与对照组相比,DM组膀胱组织变薄、逼尿肌排列紊乱、炎细胞浸润增加,而GSPE组膀胱组织较DM组增厚,逼尿肌肌肉组织排列整齐且炎细胞浸润减少。DM组膀胱组织MDA水平高于对照组,SOD、GSH-Px表达水平均低于对照组(P均<0.01)。GSPE组膀胱组织MDA表达水平低于DM组,SOD、GSH-Px表达水平均高于DM组(P<0.01或0.05)。结论 GSPE对糖尿病大鼠膀胱功能具有保护作用,其机制可能与降低膀胱组织的氧化应激损伤有关。Objective To investigate the effect and mechanism of grape-seed proanthocyanidin extract (GSPE) on bladder function of rats with diabetes mellitus (DM). Methods Thirty female Wistar rats was randomly divided into 3 groups: control group, DM group and GSPE group, and each group consisted of 10 rats. Intraperitoneal injection of streptozotocin (STZ) was used to construct the model of diabetes in DM group and GSPE group. The same dose of sodium citrate buffer was used in the control group. Rats in the GSPE group were given 250 mg/kg of GSPE by oral gavage once a day for 8 weeks. The control group and DM group were treated with the same amount of normal saline. Urodynamic method was used to examine the bladder pressure of the rats. The bladder histopathological changes were observed by HE staining. The quantity of MDA and the activities of superoxide dismutase (SOD) and GSH-Px was examined by spectrophotometry. Results Compared with the control group, the bladder contraction pressure in the DM group and GSPE group was decreased (P 〈0.01). However, the bladder contraction pressure in the GSPE group was higher than that in the DM group (P 〈 0.01 ). According to HE staining, the bladder tissue became thin, the detrusor was disordered and the infiltrating inflammatory cells were increased in the DM group as compared with those of the control group. The bladder tissue became thick and the infiltrating inflammatory cells were decreased in the GSPE group as compared with those of the DM group. Compared with the DM group, the level of MDA was lower and the levels of SOD and GSH-Px were higher in the GSPE group ( P 〈 0.01 or P 〈 0.05). Conclusion GSPE has protective effect on diabetic bladder dysfunction, and its mechanism may be associated with the increased oxidative stress injury of the bladder tissues.
