利拉鲁肽治疗多囊卵巢综合征合并2型糖尿病患者的初步临床研究

摘　　要：目的探讨利拉鲁肽与二甲双胍对多囊卵巢综合征（PCOS）合并2型糖尿病（T2DM）患者代谢和内分泌参数的影响。方法将62例PCOS合并T2DM患者随机分为两组。分别给予利拉鲁肽（1.2~1.8 mg/d）和二甲双胍0.5g/d,治疗12周。评估治疗前后各项代谢指标和性激素水平的变化。结果与治疗前相比,治疗后两组血糖（FPG）、餐后2小时血糖（2hPG）、糖化血红蛋白（HbA1c）、体质量指数（BMI）及胰岛素抵抗指数（HOMA-IR）均改善（P〈0.05）,利拉鲁肽组治疗后明显低于二甲双胍组（P〈0.05）;与治疗前相比,治疗后两组血清促黄体生成素（LH）、睾酮（T）水平明显降低（P〈0.05）。治疗前后两组性激素水平比较差异均无统计学意义（P〉0.05）。结论利拉鲁肽可改善PCOS合并T2DM患者的高血糖和胰岛素抵抗,并有助于改善高雄激素血症。Objective To investigate the efficacy of liraglutide and metformin on metabolic and endocrine parameters in polycystic ovary syndrome（PCOS）patients with type 2diabetes mellitus（T2DM）.Methods A total of62 PCOS patients with T2 DM were randomly divided into two groups,and liraglutide（1.2-1.8mg/d）or metformin（0.5g/d）were used for 12 weeks,respectively.Metabolic parameters and sexual hormones were evaluated before and after treatment.Results After 12weeks＇ treatment,the levels of FPG,2hPG,HbA1 c,BMI and HOMA-IR were significantly improved in both groups（P 0.05）,and the improvement in liraglutide group was superior than that in metformin group（P 0.05）.After treatment,the levels of LH and T were significantly reduced in both groups（P 0.05）,however,there were no significant difference in sexual hormones in pre-treatment and post-treatment（P 0.05）.Conclusion Liraglutide can improve hyperglycemia and insulin resistance in PCOS patients with T2 DM,and can also be helpful in improving hyperandrogenism.
