乳腺癌手术病人术后化疗期间生存质量的变化趋势及其影响因素

摘　　要：[目的]了解乳腺癌手术后病人在化疗期间生存质量随疗程的变化轨迹,探讨每一疗程影响生存质量的因素。[方法]采用Anderson症状调查量表、医院焦虑抑郁量表和乳腺癌病人生存质量测定量表(FACT-B)对174例乳腺癌手术后接受TAC(多西紫杉醇,阿霉素,环磷酰胺)化疗方案的病人,在其手术后化疗前和每次化疗结束后5d^7d进行问卷调查。[结果]乳腺癌术后病人的生存质量评分在手术后化疗前最差(81.2分±19.6分),第2次化疗后最好(94.5分±14.4分),第4次、第5次化疗后再次降至低谷,分别为82.7分±13.9分和82.6分±13.1分,第6次化疗后再次好转,评分为(93.9±18.7)分;并且每一疗程都有不同的相关症状影响病人的生存质量。[结论]在护理工作中应重视病人手术后化疗前及化疗第4次、第5次后生存质量的影响因素,采取干预措施治疗相关症状以提高其生存质量。Objective：To study the change track of postoperative quality of life（QOL）of patients with breast cancer during chemotherapy and to probe into the influence factors in every course.Methods：The Anderson Symptoms Survey Scale,Hospital Anxiety Depression scale（HADs）and Measurement Scale of Quality of Life for patients with Breast Cancer（FACT-B）were used to evaluate 174 patients with breast cancer who received TAC（docetaxel,doxorubicin,cyclophosphamide）chemotherapy regimen after operation,then evaluated them after surgery before chemotherapy and five to seven days after every time of chemotherapy.Results：The quality of life score of patients with breast cancer was the lowest（81.2±19.6）after surgery before operation,it was the best after the second time of chemotherapy（94.5±14.4）,it hit bottom again after the fourth time and fifth time of chemotherapy,the scores were（82.7±13.9）and（82.6±13.1）respectively,it became better again after the sixth time of chemotherapy（93.9±18.7）.In addition,there were different factors influencing the QOL of breast cancer patients in every chemotherapy course.Conclusion：In nursing work,we should pay more attention to the influence factors of the quality of life after the fourth,fifth times of chemotherapy and after surgery before chemotherapy,and to take the intervention measures to improve the quality of life.
