胆源性急性胰腺炎的早期内镜治疗临床观察

摘　　要：目的探讨早期内镜治疗胆源性急性胰腺炎的临床应用效果。方法选取2013年7月．2015年7月期间来我院治疗的100例胆源性急性胰腺炎患者作为研究对象．根据随机分组软件将100例患者随机分为观察组和对照组，每组各50例。对照组给予常规治疗，观察组给予早期内镜治疗，观察并比较两组临床治疗效果。结果观察组患者血淀粉酶、白细胞、肝功能恢复正常所用时间明显短于对照组；观察组治愈率、死亡率、住院时间优于对照组[100．0％vs96．0％，0vs4．0％，（12．6±4．8）dvs（22．7±5．2）d1，差异存在统计学意义（P〈0．05）。观察组并发症发生率为6％，对照组并发症发生率为16％，两组患者并发症发生率比较差异有统计学意义（P〈0．05）。结论对胆源性急性胰腺炎患者实施早期内镜治疗疗效显著，可有效降低因疾病死亡的机率，值得临床推广应用。Objective To discuss the clinical effect of early therapeutic endoscopy in biliary acute pancreatitis. Methods 100 patients with biliary acute pancreatitis treated in our hospital from July 2013 to July 2015 were selected and randomly divided into observation group and control group, each with 50 patients. Patients in the control group were given conventional treatment, while patients in the observation group were given therapeutic endoscopy in an early period. The clinical effects were compared between two groups. Results The recovery time of blood amylase, white blood cells, and liver function was significantly shorter in the observation group than in the control group; and the results of cure rate, mortality rate and hospital stay in the observation group were superior to those in the control group [100.0% vs 96.0%, 0 vs 4.0%, and （12.56±4.8） d vs（22.7±5.2） d], the differences were statistically significant （P〈0.05）. The incidence rate in the observation group was 6%, which was significantly lower than the 16% in the control group , the difference was statistically significant （P〈0.05）. Conclusion Early application of therapeutic endoscopy has significant effect in treatment of biliary acute pancreatitis, which can effectively reduce the mortality rate and is worthy to be promoted.
