坎地沙坦联合左旋氨氯地平对老年高血压合并糖尿病肾病的疗效观察

摘　　要：目的观察坎地沙坦酯片与左旋氨氯地平片长期联合应用治疗老年高血压伴2型糖尿病的临床疗效及对早期肾病患者尿微量白蛋白的影响。方法将中国人民解放军464医院干部病房科2011-05—2013-05收治的老年高血压伴2型糖尿病肾病的130例患者,按入院前服药情况分为单用坎地沙坦酯及单用左旋氨氯地平片两组,单用坎地沙坦酯组加用左旋氨氯地平片2.5 mg/d、单用左旋氨氯地平片组加用坎地沙坦酯8 mg/d,两组均治疗6个月,分别测定联用前、后的收缩压、舒张压、空腹血糖、餐后2 h血糖、糖化血红蛋白及尿微量白蛋白定量。结果联用后两组患者收缩压、舒张压、空腹血糖、餐后2 h血糖、糖化血红蛋白及尿微量白蛋白均较联用前明显下降,差异有统计学意义(P<0.05)。各组均未见严重不良反应。结论坎地沙坦酯片联合左旋氨氯地平片治疗老年高血压伴2型糖尿病效果明显优于单一用药,能够提高疗效,延缓疾病进程,保护肾脏,减少并发症。Objective To evaluate the clinical effect of long-term combined treatment with Candesartan and Levam- lodipine to treat senile hypertension with Type- Ⅱ diabetics and the influence on microalbuminuria in early nephrotic sufferers.Methods 130 sufferers with senile hypertension and Type-II diabetic nephropathy admitted to cadre＇s ward department of the PLA No.464 Hospital from May 2011 to May 2013 were selected and divided into two groups ac- cording to their meditation situation prior to admission.single use of Candesartan Cilexetil group and single use of Levamlodipine group. Single use of Candesartan Cilexetil group were given additional Levamlodipine for 2.5 mg/d, and single use of Levamlodipine group were given additional Candesartan Cilexetil for 8 mg/d.They were treated for six months.The systolic blood pressure, diastolic blood pressure, fasting glucose, 2 h postprandial blood glucose, glycosylat- ed hemoglobin and urinary microalbumin were measured before and after the combined treatment. Results After com- bined treatment with Levamlodipine and Candesartan, the systolic blood pressure, diastolic blood pressure, fasting glu- cose, 2 h postprandial blood glucose, glycosylated hemoglobin and urinary microalbumin in both groups clearly de- creased compared with those before the treatment,with differences of statistical significance（P 〈 0.05）.No serious ad- verse reactions were found in both groups.Conclusion The curative effect of combined treatment with Candesartan and Levamlodipine to treat senile hypertension with Type- Ⅱ diabetics is clearly superior to the single medication, which can improve curative effect, suspend diseases, protect kidneys, and reduce complication.
