摘 要:【目的】观察培土生金法(加味陈夏六君子汤)对慢性阻塞性肺疾病(COPD)模型大鼠膈肌细胞增殖与凋亡的影响。【方法】将40只Wistar大鼠随机分为空白组、模型组、中药高剂量组(71.82 g·kg-1·d-1)、中药低剂量组(17.96 g·kg-1·d-1),采用气管内注入脂多糖(LPS)联合香烟烟熏法造模,观察各组大鼠一般情况及体质量变化;苏木素—伊红(HE)染色镜下观察大鼠膈肌病理变化,免疫组织化学法检测增殖细胞核抗原(PCNA)在膈肌中的表达,并计算增殖指数(PI)。TUNEL法检测膈肌细胞凋亡情况,并计算凋亡指数(AI)及PI/AI值。【结果】与空白组比较,模型组大鼠体质量增长缓慢(P<0.01),膈肌细胞凋亡指数显著升高(P<0.01),PI/AI值显著下降(P<0.01);与模型组比较,中药高剂量组大鼠体质量增长迅速(P<0.01),细胞凋亡指数显著降低(P<0.01),PI/AI值显著升高(P<0.01)。【结论】培土生金法治疗COPD的作用与其能维持细胞增殖/凋亡平衡,减少COPD大鼠膈肌细胞凋亡有关。Objective To observe the effect of therapy of supplementing spleen to strengthen lung on the proliferation and apoptosis of the diaphragmatic muscle cells in rats with chronic obstructive pulmonary disease(COPD). Methods Forty Wistar rats were randomly divided into blank group,model group,high-dose Chinese medicine group(modified Chen Xia Liujunzi Decoction at 71.82 g·kg-1·d-1) and low-dose Chinese medicine group(modified Chen Xia Liujunzi Decoction at 17.96 g·kg-1·d-1). The rat model of COPD was established by intratracheal instillation of lipopolysaccharide(LPS) combined with exposure to cigarette smoking. The general health status and the body weight of rats were observed. Pathologic changes of diaphragm tissues were observed after hematoxylin-eosin(HE) staining. Immunohistochemistry was used to determine proliferating cell nuclear antigen(PCNA) expression in diaphragm tissues, and then we calculated the cellular proliferation index(PI). Terminal deoxynucleotidyl transferase-mediated d UTP nick- end labeling method(TUNEL)was used to examine the apoptosis of diaphragmatic muscle cells,and then we calculated the apoptotic index(AI)as well as the ratio of PI/AI. Results Compared with the blank group, the body weight of the model group was significantly decreased(P〈0.01), the AI was significantly increased(P〈0.01), and PI/AI was significantly lowered(P〈0.01). Compared with the model group,the body weight of high-dose Chinese medicine group was increased significantly(P〈0.01),AI was significantly lowered(P〈0.01),and PI/AI was significantly enhanced(P〈0.01). Conclusion The therapeutic mechanism of therapy of supplementing spleen to strengthen lung for COPD is probably related to keeping the balance between apoptosis and proliferation of diaphragm muscle cells and to the reduction of the apoptosis of diaphragm muscle cells.
