联合检测尿β_2微球蛋白与炎性因子在合并高血压2型糖尿病患者早期肾损害的应用价值

摘　　要：目的探讨尿β_2微球蛋白(β_2-MG)与超敏C-反应蛋白(hs CRP)、白细胞介素-6(IL-6)等炎性因子在合并高血压2型糖尿病患者早期肾损害的关系及其应用价值。方法测定125例合并高血压2型糖尿病患者和75例健康者(正常对照组)尿β_2-MG、hs CRP、IL-6、血清尿素氮(BUN)、肌酐(SCr)、糖化血红蛋白(Hb Alc)水平,并对检测结果进行统计学处理。结果糖尿病组与正常对照组比较在血清BUN、SCr正常范围时其尿β_2-MG、血清hs CRP、IL-6水平均已升高,差异有统计学意义(P<0.05),且尿β_2-MG与血清hs CRP、IL-6水平呈正相关;Hb Alc水平越高的患者尿β_2-MG、血清hs CRP、IL-6升高越明显。结论不少合并高血压的糖尿病患者BUN、SCr虽然正常,但其肾功能已经下降,尿β_2-MG、血清hs CRP、IL-6与糖尿病早期肾损害有着更良好的相关性,联合检测可明显提高糖尿病肾病诊断符合率,对合并高血压2型糖尿病早期肾损害有更高的临床诊断价值,利于临床早期及时干预。Objective To investigate relationships and diagnosis value of urine β_2- microglobulin（β_2-MG）,plasma high sensitive C-reactive protein（hs CRP）and interleukin-6（IL-6）such inflammatoryfactors in early renal damage of type-2 diabetic patients with hypertension. Methods Urine and plasmasamples were collected from 125 type-2 diabetic patients with hypertension and 75 healthy volunteers,and urine β_2- MG,hs CRP,IL- 6,BUN,SCr,Hb Alc of the two groups were tested and compared.Results Urine β_2-MG,plasma hs CRP and IL-6 levels of diabetic patients group were higher than those ofthe healthy group when BUN and SCr were normal range,the difference was statistically significant（P 0. 05）.There was a positive correlation between urine β_2- MG and plasma hs CRP,IL- 6 of diabeticpatients. The higher the Hb Alc level, the higher the urine β_2- MG, plasma hs CRP and IL- 6.Conclusion A considerable proportion of diabetic patients with hypertension have low renal functionwhile their BUN,SCr are still normal,Urine β_2-MG and plasma hs CRP,IL-6 have a good correlationwith early renal damage of diabetic patients with hypertension. It is suggested that combined detection canbenefit the diagnosis of early renal damage in such patients,and is in favor of early intervention.
