摘　　要：目的：分析噻托溴铵干粉与沙美特罗替卡松粉剂联合应用对稳定期慢性阻塞性肺疾病（COPD）患者病情控制的效果。方法：将2012年9月～2015年6月间在延安大学附属医院接受治疗的COPD患者116例纳入研究,根据相关检测确定以上研究对象均处于稳定期。根据接受的治疗方式不同,所有入组对象被分为观察组及对照组各58例。对照组患者接受临床常规治疗,观察组患者接受噻托溴铵干粉与沙美特罗替卡松粉剂联合治疗,对比两组患者的治疗后血清及诱导痰中病情相关指标水平。结果：观察组患者接受治疗后的诱导痰γ-GCS、MIP-1α、8-iso-PGF2、HMGB1、IL-17、Hbd-2值低于对照组患者,SP-A值高于对照组患者（均P〈0.05）;观察组患者的治疗后FEF25%～75%、FEV1%、V25、MMFR值高于对照组患者,血清CXCR3、CXCL10值低于对照组患者（均P〈0.05）;观察组患者的治疗后血清和肽素、锁链素、CCL18、CC16、ANG2值均低于对照组患者（均P〈0.05）。结论：稳定期COPD患者接受噻托溴铵干粉与沙美特罗替卡松粉剂联合治疗,有助于进一步稳定患者病情并改善预后。Objective：To analyze the effect of combined application of tiotropium and salmeterol and fluticasone propionate powder on stable COPD patients.Methods：A total of 116 COPD patients who received treatment in our hospital from September 2012 to June 2015 were included in the research and confirmed to be in stable phase according to relevant detection.According to different treatment methods they received,all included subjects were divided into observation group and control group by half.Control group received clinical conventional treatment,observation group received combined treatment of tiotropium and salmeterol and fluticasone propionate powder,and then the levels of illness-related indicators in serum and induced sputum were compared between two groups after treatment.Results：γ-GCS,MIP-1α,8-iso-PGF2,HMGB1,IL-17 and Hbd-2values in induced sputum of observation group after treatment were lower than those of control group while SP-A value was higher than that of control group（P〈0.05）;FEF25%-75%,FEV1%,V25 and MMFR values of observation group after treatment were higher than those of control group while serum CXCR3 and CXCL10values were lower than those of control group（P〈0.05）;serum copeptin,desmosine,CCL18,CC16 and ANG2values of observation group after treatment were lower than those of control group（P〈0.05）.Conclusions：Combined treatment of tiotropium and salmeterol and fluticasone propionate powder helps to further stabilize COPD patients＇ condition and improve the prognosis.
