踝关节骨折合并三角韧带损伤手术中修复三角韧带与不修复对于预后的影响

摘　　要：目的探讨踝关节骨折合并三角韧带损伤手术中修复三角韧带与不修复对于预后的影响。方法选择2008年8月～2014年12月在我院诊治的踝关节骨折合并三角韧带损伤患者96例,根据随机数字表法分为治疗组与对照组各48例,治疗组在手术中行三角韧带修复,对照组在手术中不修复三角韧带。之后对两组患者围手术指标,功能及疼痛评价以及并发症发生情况进行对比。结果两组的手术时间和术中出血量对比差异无统计学意义（P〉0.05）,治疗组术后住院时间明显少于对照组（p〈0.05）。术后3个月治疗组踝关节功能优良率为95.8%,对照组为83.3%,治疗组优良率明显高于对照组（P〈 0.05）。两组术后3个月疼痛评分分别为（1.04±0.34）分和（2.78±0.45）分,都明显低于术前（6.22±0.67）分和（6.19±0.44分）,（p〈0.05）,组间对比差异都有统计学意义（P〈 0.05）。术后3个月治疗组切口出血、肺部感染、切口感染、肿胀等并发症发生情况明显少于对照组（P〈 0.05）。结论踝关节骨折合并三角韧带损伤手术中修复三角韧带能促进踝关节功能的恢复,能缓解疼痛,减少术后并发症,有很好的应用效果。Objective To investigate the prognosis effects of the deltoid ligament repair does not or do for the ankle fracture combined with triangular ligament patients. Methods From August 2008 to December 2014 in our hospital, selected 96 ankle fracture combined with triangular ligament patients, accorded to the random number table were equally divided into the treatment group and the control group of 48 patients, and the treatment group were given the triangle ligament repair, the control group were not given the triangle ligament repair. Then the perioperative indexes, function and pain value and the complications were observed and compared. Results The operative time and blood loss compared between the two groups were no significant differences（p〉 0.05）, the postoperative hospital stay in treatment group were significantly less than in the control group（p〈 0.05）. The postoperative three months of ankle function excellent in the treatment group and the control group were 95.8% and 83.3% that the treatment group were significantly higher（p〈 0.05）. The postoperative three months pain score in the treatment group and control group were（1.04±0.34） points and（2.78±0.45） points that were significantly lower than the preoperative of（6.22±0.67）points and（6.19±0.44）points（p〈 0.05）, while compared the groups differences were statistically significant（p〈 0.05）. The postoperative three months incision bleeding, pulmonary infection, wound infection, swelling and other complications in the treatment group were significantly less than the control group（p〈 0.05）.Conclusion The deltoid ligament repair do for the ankle fracture combined with triangular ligament patients can contribute to the restoration and rehabilitation of patients with ankle joint function, it can relieve the patient＇s pain and reduce the incidence of postoperative complications, there has very good application effects.
