摘  要:目的:观察干预肾素-血管紧张素-醛固酮系统不同环节对单侧输尿管梗阻大鼠肾组织的影响。方法:选取6周龄SD大鼠50只作为研究对象,随机分为正常组、模型组、依那普利组、缬沙坦组及螺内酯组,每组10只。采用单侧输尿管梗阻(UUO)大鼠模型,正常组和模型组于造模前24 h-造模后14 d给予0.9%氯化钠溶液10 m L/(kg·d)灌胃,依那普利组、缬沙坦组及螺内酯组分别于造模前24 h-造模后14 d给予依那普利10 mg/(kg·d)、缬沙坦10 mg/(kg·d)、螺内酯100 mg/(kg·d)灌胃。各组大鼠均于造模后2周处死,收集血、尿标本进行生化检测,肾组织行HE染色及逆转录聚合酶链反应(RT-PCR)法观察梗阻肾组织TGF-β1 m RNA、血管紧张素原(AGT)m RNA、血管紧张素转化酶(ACE)m RNA、醛固酮(ALD)m RNA的表达。结果:与正常组比较,模型组、依那普利组、缬沙坦组及螺内酯组的血肌酐、尿素氮均不同程度增高,以模型组升高最为明显,比较差异均有统计学意义(P<0.05),依那普利组、缬沙坦组及螺内酯组间比较差异无统计学意义(P>0.05);五组间24 h尿蛋白比较差异均无统计学意义(P>0.05);依那普利组、缬沙坦组及螺内酯组肾间质损伤指数均高于正常组,但低于模型组,比较差异均有统计学意义(P<0.05)。结论:阻断肾素-血管紧张素-醛固酮系统不同环节均具有抑制大鼠肾间质纤维化的作用,且其效果相近。Objective:To observe the effects of different aspects of the intervention of the renin angiotensin aldosterone system on the renal tissue of rats with unilateral ureteral obstruction.Method:A total of 50 SD rats which was 6 weeks old were selected as the research objects,they were randomly divided into the normal group,model group,Enalapril group,Valsartan group and Spironolactone group,each group had 10 rats.The rat model of unilateral ureteral obstruction(UUO) was used,the normal group and model group were given saline 10 mL/(kg·d) 0.9% Sodium Chloride Solution before 24 h for modeling to 14 d after modeling;Enalapril group,Valsartan group and Spironolactone group 24 h for modeling to 14 d after modeling were respectively treated with Enalapril 10 mg/(kg·d),Valsartan 10 mg/(kg·d),Spironolactone 100 mg/(kg·d),rats in each group were sacrificed at 2 weeks after modeling,blood and urine samples were collected for biochemical tests,the obstructive renal tissues TGF-β1 m RNA,angiotensinogen(AGT) m RNA,vascular tension angiotensin converting enzyme(ACE) m RNA and aldosterone(ALD) m RNA expression were observed according renal tissue for HE staining and reverse transcriptase polymerase chain reaction(RT-PCR) method.Result:Compared with normal group,the serum creatinine and urea nitrogen were increased in the model group,enalapril group,Valsartan group and Spironolactone group,model group increased most significantly,the differences were statistically significant(P〈0.05),there were no statistically significant differences in the Enalapril group,Valsartan group and Spironolactone group(P〉0.05). There were no significant differences in five groups of 24 h urine protein(P〉0.05). The renal interstitial injury index of Enalapril group,valsartan group and Spironolactone group were higher than normal group,but lower than model group,the differences were statistically significant(P〈0.05).Conclusion:Blockade of the rennin-angiotensin-aldosterone system at
