摘  要:目的:观察头针电刺激对不同时间点脑梗死大鼠Bax与Bcl-2 mRNA的变化,探讨其对脑梗死可能作用机制。方法:采用线栓法制作大鼠局灶性脑梗死模型。将180只健康Wistar雌性大鼠,随机分为3组,头针电刺激组、模型对照组和假手术组。各组又依据脑梗死后治疗时间点不同分为4个亚组,即1天组、3天组、7天组与14天组。3组大鼠造模结束24 h起,给予头针电刺激组大鼠针刺百会穴与大椎穴,并进行电刺激治疗,1次/日,30 min/次,余2组在每天相应时间固定大鼠于针刺台,不予任何处置。疗程治疗结束后,断头取脑。运用RT-PCR检测梗死脑组织Bax与Bcl-2 mRNA。3组大鼠在造模结束清醒后、每日治疗前后均要进行神经功能缺损程度评分,采用Zea-longa 5级评分标准。结果:头针电刺激组中7天与14天的亚组大鼠Zea-longa 5级评分明显低于同组1天与3天的亚组(P<0.05),14天的Zea-longa 5级评分与模型组比较,具有显著性差异(P<0.05);头针电刺激组经RTPCR检测后,Bax mRNA的表达逐渐降低,Bcl-2逐渐增加,且14天的亚组与模型组14天比较,有显著性差异(P<0.05),与假手术组14天比较则无显著性差异(P>0.05)。结论:头针电刺激能明显降低脑梗死大鼠的神经功能缺损程度评分,改善大鼠运动功能;头针电刺激能降低Bax mRNA的表达,提高Bcl-2 mRNA的表达,从而抑制缺血区细胞凋亡,保护脑组织,促进肢体康复的功效,其中以头针电刺激14天疗效最佳。Objective: To observe the effect of scalp electrical stimulation at different time points on Bax and Bcl- 2 mRNA changes in cerebral infarction rats,and to explore its possible mechanism. Methods: The rat models of focal cerebral infarction were induced by suture method. The 180 healthy female wister rats were randomly divided into three groups as a scalp electrical stimulation group,a model control group and a sham group. Each group based on the treatment time point after cerebral infarction was divided into four subgroups,namely 1 d group,3 d group,7 d group and 14 d group. Since the end of 24 h of rats modeling in three groups,give scalp electric acupuncture at Baihui and Dazhui in stimulation group,with electrical stimulation therapy,1 time / day,30 min / time,but rats in other two groups in the corresponding time every day were fixed at acupuncture units,without any disposal. After the appropriate course of treatment,the brains were removed. Use RT- PCR to detect Bax and Bcl- 2 mRNA of infarcted brain tissue. After rats were sober at the end of modeling in three groups,neurological impairment score before and after the daily treatment was carried out,using Zea- longa five scoring criteria. Results: Zea- longa five scores in the 7 d and 14 d of the subgroups of scalp electrical stimulation group were significantly lower than those in the 1 d and 3 d subgroups( P〈0. 05),Zea- longa five scores in 14 d group had a significant difference compared with the model group( P〈0. 05); in scalp electrical stimulation group after RT- PCR the expression of Bax mRNA was gradually decreased,but Bcl- 2 was gradually increased,and the 14 d sub- group was compared with the 14 d model group,with significant differences( P〈0. 05),but compared with the 14 d sham group,there was no significant difference( P〈0. 05).
