不同固定方式治疗胸腰段脊柱骨折的效果比较

摘　　要：目的探讨不同固定方式治疗胸腰段脊柱骨折的效果比较。方法选择2014年1～11月在我院骨科就诊的96例胸腰段脊柱骨折患者作为研究对象,根据随机数字表法将其分为内固定组和内固定＋植骨组,每组48例。内固定组以椎弓根钉内固定术治疗;内固定＋植骨组以椎弓根钉内固定术＋椎板间植骨治疗。比较两组患者的脊髓神经功能优良率、治疗前后凸Cobb角、椎管内占位、伤椎前缘高度。结果两组患者治疗后凸Cobb角显著减小、椎管内占位显著降低,伤椎前缘高度显著提高,且内固定＋植骨组改善幅度更大,组间数据比较差异有统计学意义（P〈0.05）。内固定＋植骨组患者脊髓神经功能优良率明显高于内固定组,差异有统计学意义（P〈0.05）。结论椎弓根钉内固定术＋椎板间植骨治疗胸腰段脊柱骨折临床效果确切,可改善脊髓神经功能,减小凸Cobb角、降低椎管内占位,提高伤椎前缘高度,值得推广。Objective To explore the effect of different fixation type in treating thoracolumbar spinal fractures. Methods 96 thoracolumbar spinal fractures patients from January to November 2014 of our orthopedics department were selected as study objects and were divided into internal fixation group and internal fixation plused bone grafting group according to the random number table method.Each group was 48 cases.Patients in internal fixation group were given internal transpedicular fixation,and patients in internal fixation plused bone grafting group were given internal transpedicular fixation combined with bone grafting between vertebral plate.Good rate of spinal cord nerve function,convex Cobb angle before and after treatment, intraspinal space occupied,anterior vertebral height in two groups were compared.Results After treatment,both of convex Cobb angle and intraspinal space occupied obvious decreased between two groups and anterior vertebral height obvious increased,and the improvement degree in internal fixation plused bone grafting group was more and the difference between two groups was statistical significance（P〈0.05）.The good rate of spinal cord nerve function in internal fixation plused bone grafting group was obvious higher than that in the internal fixation group and the difference between two groups was staitistical significance（P〈0.05）. Conclusion Internal transpedicular fixation combined with bone grafting between vertebral plate has definite effect in treating thoracolumbar spinal fractures.It can improve spinal cord nerve function,decrease convex Cobb angle and intraspinal space occupied and enhance anterior vertebral height.It is worth to popularize.
