动力髋螺钉与股骨近端防旋髓内钉治疗老年股骨转子间骨折对围术期失血量的影响

摘　　要：目的分析动力髋螺钉（DHS）与股骨近端防旋髓内钉（PFNA）治疗老年股骨转子间骨折围术期失血量。方法回顾性分析2009年7月～2015年7月我院获得随访的86例患者资料,按手术类型将其分为DHS组和PFNA组,比较两组患者的术中失血量、术后失血量、隐性失血量等。结果 DHS组和PFNA组术中失血量分别为（162.5±30.6）、（133.6±41.5）ml,术后失血量分别为（215.7±46.8）、（326.9±55.2）ml,隐性失血量分别为（426.7±46.8）、（596.9±55.2）ml,两组以上指标比较,差异有统计学意义（P〈0.05）。结论与DHS比较,PFNA治疗老年股骨转子间骨折术中创伤小、出血小,但术后失血量及隐性失血量均较DHS大,应予以重视。Objective To analyze the amount of bleeding during perioperative period due to intertrochanteric fracture in elderly patients treated by dynamic hip screw（DHS） and proximal femoral nail anti-rotation（PFNA）. Methods From July2009 to July 2015,data from 86 patients during follow-up visits were retrospectively analyzed and divided into DHS group and PFNA group according to the type of surgery.The amount of bleeding during surgery as well as obvious and hidden blood loss after surgery between the two groups was compared. Results The intraoperative amount of bleeding in DHS group and PFNA group was（162.5±30.6） ml and（133.6±41.5） ml respectively.The postoperative obvious blood loss in the two groups was（215.7±46.8） ml and（326.9±55.2） ml and the hidden blood loss after surgery in both groups was（426.7±46.8） ml and（596.9±55.2） ml.The indexes mentioned above were all displayed statistical differences（P〈0.05）. Conclusion Compared with DHS,intertrochanteric fracture treated by PFNA has small trauma and little blood loss,but larger obvious and hidden amount of bleeding after surgery,which should be paid attention.
