淋巴细胞主动免疫联合黄体酮治疗原因不明复发性流产的疗效分析

摘　　要：目的探讨淋巴细胞主动免疫联合黄体酮治疗原因不明复发性流产的疗效。方法选取本院2014年9月～2015年9月收治的72例不明原因复发性流产患者作为研究对象,根据治疗方式的不同分为对照组（32例）和联合治疗组（40例）。对照组怀孕前不做任何形式的治疗,妊娠后肌内注射人绒毛膜促性腺激素;另外,每天肌内注射40 mg黄体酮。联合治疗组采用淋巴细胞主动免疫联合黄体酮治疗。比较两组治疗前后的干扰素-γ（IFN-γ）、白介素-4（IL-4）水平以及妊娠成功率、流产率。结果联合治疗组治疗后的IFN-γ水平显著低于治疗前及对照组治疗后,IL-4水平显著高于治疗前及对照组治疗后,差异有统计学意义（P〈0.05）。联合治疗组的妊娠成功率显著高于对照组,流产率显著低于对照组,差异有统计学意义（P〈0.05）。结论淋巴细胞主动免疫联合黄体酮治疗原因不明复发性流产效果显著,值得临床推广应用。Objective To explore the effect of lymphocyte immune actively combined with progestin in the treatment of unexplained recurrent spontaneous abortion. Methods 72 cases of unexplained recurrent spontaneous abortion from September 2014 to September 2015 in our hospital were selected and divided into the control group（32 cases） and the combination treatment group（40 cases） according to different treatment methods.The control group was not given any treatment before pregnancy,intramuscular injection of human chorionic gonadotropin was given after pregnancy.In addition,intramuscular injection of 40 mg progestin in every day,the combination treatment group was given lymphocyte immune actively combined with progestin.The level of IFN-γ and IL-4 before and after treatment in the two groups was compared,and the pregnancy success rate and abortion rate in the two groups was compared. Results The level of IFN-γafter treatment in the combination treatment group was lower than that before treatment and in the control group after treatment,the level of IL-4 after treatment in the combination treatment group was higher than that before treatment and in the control group after treatment,with significant difference（P〈0.05）.The pregnancy success rate in the combination treatment group was higher than that in the control group,the abortion rate in the combination treatment group was lower than that in the control group,with significant difference（P〈0.05）. Conclusion The effect of lymphocyte immune actively combined with progestin in the treatment of unexplained recurrent spontaneous abortion is significant,it is worthy of clinical promotion and application.
