结核感染T细胞斑点试验对肺结核并发结核性心包炎患者的诊断价值

摘　　要：目的:探讨肺结核并发结核性心包炎进行结核感染T细胞斑点试验的诊断价值。方法:选取32例疑似肺结核并发结核性心包炎患者作为观察组,选取同期32例无肺结核原病灶的心包积液患者作为对照组,分析T细胞斑点试验的诊断结果。结果:1检验结果:64例患者结核菌素试验:阳性44例、阴性22例;结核抗体检测:阳性29例、阴性35例;腺苷脱氨酶试验:阳性28例、阴性36例;T细胞斑点试验:阳性31例、阴性33例。2对照组阳性检出率均明显低于观察组,阴性检出率均明显高于观察组,均有统计学意义(P<0.05)。3T细胞斑点试验的阳性检出率明显高于其他三种检测方式,差异有统计学意义(P<0.05)。4T细胞斑点试验的阴性预测值、阳性预测值、特异度、敏感度分别为90.91%、93.55%、93.75%、90.63%,明显高于其他三种检测方式,差异有统计学意义(P<0.05)。结论:T细胞斑点试验诊断肺结核并发结核性心包炎有较高的应用价值。Objective To investigate the diagnosis value of T cell dot test of tuberculosis infection in pulmonary tuberculosis patients complicated with tuberculous pericarditis. Methods The clinical data of 32 cases of suspected pulmonary tuberculosis complicated with tuberculous pericarditis in our hospital from as the observation group,and 32 cases of patients with primary focus of non tuberculosis in the same period as the control group,the diagnostic results of T cell spot test were analyzed. Results ①Test results： in 64 patients,tuberculin test： 44 cases were positive and negative in 22 cases; tuberculosis antibody detection： positive in 29 cases,35 cases were negative; adenosine deaminase test： 28 cases were positive,36 cases were negative; T cell spot test： 31 cases were positive,33 patients with negative. ②The positive detection rate of the observation group was lower than that of the control group,the negative detection rate was higher than that in the observation group,and the comparison was statistically significant（ P〈0. 05）. ③The positive detection rate of T cell spot test was significantly higher than that in the other three detection methods,and the correlation was statistically significant（ P〈0. 05）. ④T cell spot test negative predict value,positive predictive value,specificity,sensitivity was respectively 90. 91%,93. 55%,93. 75% and 90. 63%,was significantly higher than other three kinds of detection methods,data contrast had obvious difference,there were statistical significance（ P〈0. 05）. Conclusion T cell spot test in the diagnosis of pulmonary tuberculosis complicated with tuberculosis pericarditis with high application value,is conducive to achieving the ideal effect of diagnosis,it is worth of clinical application.
