宣白承气汤联合西药治疗气虚型急性肺损伤随机平行对照研究

摘　　要：[目的]观察宣白承气汤联合西药治疗气虚型急性肺损伤疗效。[方法]使用随机平行对照方法,将56例住院患者按掷骰子法简单随机分为两组。对照组28例机械通气,维持水、电解质和酸碱平衡,抗感染、祛痰、抑制炎症等;开塞露40m L+9%250m L生理盐水,灌肠,1次/d。治疗组28例宣白承气汤(生石膏15g,苦杏仁、瓜蒌皮、生大黄各10g),1剂/d,水煎400m L,早晚口服,200m L/次;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、PIP、PP、PEEP、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效16例,有效9例,无效3例,总有效率89.29%。对照组显效11例,有效7例,无效10例,总有效率64.29%。治疗组疗效优于对照组(P<0.05)。PIP、PP、PEEP两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]宣白承气汤联合西药治疗气虚型急性肺损伤疗效满意,无严重不良反应,值得推广。[Objective] To observe Xuanbai Cheng Qi Tang combined with western medicine ALI effect. [Methods] randomized, parallel controlled method, 56 cases of hospitalized patients were randomly divided into two groups craps method is simple. The control group of 28 patients with mechanical ventilation, maintenance of water, electrolyte and acid-base balance, anti-infective, expectorant, suppress inflammation. Enema 40 m L＋9% 250 m L saline enema, 1 times/d. Treatment group, 28 patients Xuanbai Cheng Qi Tang（Shengshigao 15 g, Kuxingren,Gualoupi,Shengdahuang each 10g）, 1/d, decoction 400 m L, sooner or later, oral, 200 m L/second; western medicine with the control group. 7d for a course of continuous treatment. Observation of clinical symptoms, PIP, PP, PEEP, adverse reactions. One course of treatment, to determine efficacy. [Results] The treatment group 16 cases, effective in 9 cases, 3 cases, the total efficiency of 89.29%. In the control group, 11 cases markedly effective in 7 cases, 10 cases, the total efficiency of 64.29%. Treatment group than the control group（P〈0.05）. Breathing index improved in both groups（P〈0.01）, the treatment group than the control group improved（P〈0.01）. [Conclusion] Xuanbai Cheng Qi Tang combined with western medicine ALI Results were satisfactory, no serious adverse reactions, is worth promoting.
