Gamma钉和DHS治疗股骨粗隆间骨折的应用分析

摘　　要：目的 探讨Gamma钉、动力髋螺钉（DHS）治疗股骨粗隆间骨折的临床疗效、安全性、并发症以及术中术后注意事项.方法 125例股骨粗隆间骨折患者根据治疗方法不同分为Gamma钉组（57例）与DHS组（68例）.分别应用Gamma钉、DHS内固定,观察两组临床疗效、安全性以及并发症情况.结果 所有患者无一例死亡、无一例深静脉血栓形成以及切口感染;Gamma钉组切口脂肪液化2例、DHS组3例;Gamma钉组术后肺部感染3例、DHS组2例;所有病例均骨性愈合,愈合时间Gamma钉组11~20周（平均15周）、DHS组14~22周（平均16周）;髋关节功能优良率分别为93.0%和92.6%,两组比较差异无统计学意义（P〉0.05）;随访时间Gamma钉组为6~22个月（平均15个月）、DHS组6~26个月（平均16个月）.结论 Gamma钉、DHS是治疗股骨粗隆间骨折非常有效的内固定方式,临床疗效肯定,安全性高,并发症少且无显著差异,值得推广.Objective To investigate clinical effects, safety, complications, intraoperative and postoperative key points of Garrnna nail and dynamic hip screw （DHS） in the treatment of femoral intertrochanteric fracture. Methods A total of 125 patients with femoral intertrochanteric fracture were divided by different treatment way into Gamma nail group （57 cases） and DHS group （68 cases）, and they received respectively Gamma nail and DHS for internal fixation. Clinical effects, safety and complications were observed between the two groups. Results There was no death ease, deep venous thrombosis or incision infection case. There were 2 cases with incision fat liquefaction in the Gamma nail group and 3 eases in the DHS group. There were 3 cases with postoperative pulmonary infection in the Gamma nail group and 2 cases in the DHS g（oup. All cases had bony union, with the healing time as 11-20 weeks （15 weeks as average） in the Gamma nail group and 14-22 weeks （16 weeks as average） in the DHS group. Good rates of hip joint function were respectively 93.0% and 92.6% in the two groups, and the difference had no statistical significance（P〉0.05）. The Gamma nail group had follow-up for 6-22 months （average time as 15 months） and the DHS group had it for 6-26 months （average time as 16 months）. Conclusion Gamma nail and DHS are effective internal fixation measures in treating femoral intertrochanterie fracture. They are worth promoting due to precise clinical effect, high safety, and few complications, without obvious difference.
