盐酸吗啡缓释片与硫酸吗啡缓释片治疗重度癌性疼痛的疗效比较

摘　　要：目的:比较盐酸吗啡缓释片与硫酸吗啡缓释片治疗重度癌性疼痛的有效性及安全性。方法:选取濮阳市人民医院2013年5月—2014年5月收治的128例重度癌性疼痛患者作为研究对象,按照随机数字表法分为对照组和观察组,每组各64例,对照组患者应用盐酸吗啡缓释片,观察组患者应用硫酸吗啡缓释片,观察2组患者的疼痛强度评分、疗效及不良反应发生情况。结果:2组患者治疗1、3、5、7 d后的疼痛强度评分均较治疗前显著降低,差异均有统计学意义(P<0.05)。观察组患者的总有效率为90.63%(58/64),对照组为89.06%(57/64),差异无统计学意义(P>0.05);观察组患者恶心、呕吐、便秘发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论:硫酸吗啡缓释片治疗重度癌性疼痛的临床疗效与盐酸吗啡缓释片相近,患者均可耐受,但对于有消化道疾病史、年龄较大者,应用硫酸吗啡缓释片的安全性更好。OBJECTIVE：To investigate the efficacy and safety of Morphine hydrochloride sustained-release tablets and Morphine sulfate sustained-release tablets in treatment of severe cancer pain.METHODS：128 cases of severe cancer pain admitted into Puyang Municipal People＇s Hospital from Mar.2013 to Apr.2014 were selected to be divided into observation group and control group via the random number table,with 64 cases in each.The control group were treated with Morphine hydrochloride sustained-release tablets,while the observation group received Morphinesulfate sustained-release tablets,and observed the pain intensity ratings,curative effect and adverse reactions occur in two groups.RESULTS：After treatment of 1 d,3 d,5 d and 7 d,the pain intensity score of two group of patients were significantly decreased compared with before treatment,the difference was statistically significant（P 〈0.05）.The total effective rate of observation group was 90.63%（58 /64）,significantly lower than that of control group [89.06%（57 /64） ],the difference was not statistically significant in two groups（P 〉0.05）.The incidence of nausea,vomiting,constipation and adverse reactions in observation group were significantly lower than that of control group,with statistically significant difference（P〈 0.05）.CONCLUSIONS：The clinical efficacy of Morphine sulfate sustainedrelease tablets and Morphine hydrochloride sustained-release tablets in treatment of severe cancer pain are similar,the adverse reactions can be tolerated by patients,yet the security of Morphine sulfate sustained-release tablets for patients with a history of gastrointestinal disease,older age would be better.
