建德市17家基层卫生院门、急诊抗菌药物使用合理性分析

摘　　要：目的:了解建德市(以下简称"我市")基层卫生院门、急诊抗菌药物使用情况,评价其合理性,为抗菌药物的合理应用和规范管理提供科学依据。方法:抽查我市17家基层卫生院2015年6月的门、急诊抗菌药物处方各100张,共1 700张,按照建立的门、急诊处方抗菌药物使用合理性评价标准,对抽查处方抗菌药物的使用情况进行合理性分析。结果:抗菌药物注射给药率为60.71%(1 032/1 700);抗菌药物二联用药442例(占26.00%),三联用药12例(占0.71%);抗菌药物不合理使用处方488张(占28.71%),其中"使用方法不合理"的处方占所有不合理处方的50.82%(248/488)。结论:我市基层卫生院门、急诊使用抗菌药物在用药指征、品种选择、使用方法、联合用药等各方面均存在明显的问题,因此,必须加大监管力度,采取多项切实有效的管理措施,规范基层卫生院抗菌药物的使用,提高合理用药水平。OBJECTIVE：To investigate the application of antibiotics in outpatient and emergency department of 17 primary hospitals in Jiande（hereinafter referred to as ＂our city＂）,so as to provide scientific basis for the rational use of antibiotics and standardize management.METHODS：100 antibiotic prescriptions each were randomly selected from outpatient and emergency department of 17 primary hospitals in Jun.2015,totally 1 700 prescriptions.According tothe rationality evaluation criteria established by outpatient and emergency department,analysis was conducted on the application rationality of antibiotics of the sampled prescriptions.RESULTS：The administration rate of antibiotics was60.71%（1 032 /1 700）;442 cases were two-drug of antibiotics（26.00%）,12 cases were three-drug of antibiotics（0.71%）.Prescriptions of irrational use of antibiotics were 488 cases（28.71%）,among which,the ratio of prescriptions with irrational methods was 50.82%（248 /488）.CONCLUSIONS：Significant problems still exist in the medication indications,variety selection,application methods and drug combination in the primary hospitals of our city,it is necessary to strengthen the supervision,take various effective management measures and standardize the application of antibiotics in primary hospital,so as to improve the level of rational drug use.
