摘　　要：目的:了解复旦大学附属妇产科医院(以下简称"我院")门、急诊的用药情况,促进临床合理用药,为开展妇产科临床药学服务提供参考。方法:依据《医院处方点评管理规范(试行)》和《2011年全国抗菌药物临床应用专项整治活动方案》的要求,按等间隔方式,从我院2015年1—6月每周门、急诊处方中随机抽取不少于处方总数25%的抗菌药物处方和不少于100张的普通处方,对其进行点评分析。结果:共抽取14 532张处方,不合理处方共有204张,不合理处方率占1.40%。其中,不规范处方137张,占不合理处方数的67.16%;用药不适宜处方65张,占不合理处方数的30.39%;超常处方2张,占不合理处方数的0.98%。结论:我院门、急诊合理处方率为98.60%,但仍存在少量问题,建议加强对医师处方书写、合理用药知识培训,规范医疗行为;提升药师专业水平,进一步加强处方审核;加强临床药师参与临床查房,对临床医师用药提出合理化建议。OBJECTIVE：To investigate the drug use status in outpatient and emergency department of Obstetrics and Gynecology Hospital Affiliated to Fudan University（hereinafter referred to as ＂our hospital＂）,so as to promote rational drug use in clinic and provide reference for the development of gynecological and obstetrical clinical pharmacy services.METHODS：According to the principle of Hospital Prescription Review Management Specification（trial） and Clinical Application of Antibiotics Rectification Activities,≥ 25% weekly outpatient and emergency antibacterial prescriptions and ≥ 100 general prescriptions from Jan.2015 to Jun.2015 were randomly sampled and then evaluated.RESULTS：Of the total 14 532 prescriptions,204 prescriptions were irrational,and the irrational rate was1.40%;among which,137（67.16%） were non-standard prescriptions,65（30.39%） were inappropriate prescriptions,and 2（0.98%） were off-label drug use.CONCLUSIONS：The rational rate of outpatient and emergency prescriptions in our hospital was 98.60%,yet there are still some problems.It is advisable to strengthen the clinicians＇ ability in prescription writing and knowledge in rational drug use,and regulate medical practices.Through enhancing the professional standards of pharmacists,strengthening prescription audit,improving the participation of clinical pharmacists in clinical ward round,rationalization proposal on drug use would be put forward for clinicians.
