摘　　要：目的：探讨micrRNA-21（miR-21）在川崎病（KD）患者血清中的表达及其临床意义。方法：收集温州医科大学附属第二医院育英儿童医院2014年1月-2015年6月收治的33例KD患者以及15例健康体检者的血清标本,以cel-miR-39为外参基因,通过实时定量PCR（RT-q PCR）检测血清miR-21的表达,并通过受试者工作特征（ROC）曲线评估其作为KD疾病标记物的诊断价值。结果：miR-21在KD急性期患儿血清中表达水平明显高于健康儿童血清（P〈0.05）,恢复期则降低至正常水平。ROC曲线显示,以miR-21诊断KD的曲线下面积（AUC）为0.736（95%CI：0.531~0.941）,当临界值（诊断阈值）取1.09时,其诊断灵敏度和特异度分别为0.846和0.714。血清miR-21表达与KD临床特征相关分析显示,高血小板水平血清中miR-21表达更高。结论：KD患者血清miR-21水平显著升高,可能是KD诊断的潜在血清标志物之一。Objective： To study the expression of miR-21 in the serum of patients with Kawasaki disease and its clinical signifi cance. Methods： Thirty-three cases of Kawasaki disease and 15 healthy control were collected from the Second Affiliated Hospital Yuying Children＇s Hospital of Wenzhou Medical University in 2014 and 2015. With cel-miR-39 as the external reference gene, the expression of miR-21 was detected with realtime quantitative PCR, and its signifi cance as a marker in diagnosis was evaluated by ROC curve. Results： The expression of miR-21 in serum of KD patients was signifi cantly higher than that in the healthy control group（P〈0.05）, and return to normal level after treatment. ROC curve indicated miR-21 had a sensitivity of 84.6% and a specifi city of 71.4% for distinguishing KD patients from healthy children with an area under the curve（AUC） were 0.736（95%CI： 0.531~0.941）. Further analysis of the clinical pathological features indicated that the level of serum miR-21 was associated with PLT. Conclusion： Serum miR-21 level is signifi cantly elevated in KD patients, it may be used as a potential biomarker in diagnosis of KD.
