精细护理在中西医结合治疗开普拓所致腹泻中的应用效果

摘　　要：目的探讨精细护理在中西医结合治疗开普拓所致化学性腹泻中的应用效果。方法按随机数字表将60例患者均分为实验组和对照组,实验组治疗前1天口服加味黄芩汤,对照组行常规治疗,观察2组腹泻发生情况及程度,并比较骨髓抑制、恶心呕吐、食欲减退等不良反应发生率。将实验组患者随机均分为干预组和空白组,空白组实施常规护理,干预组实施精细护理,从心理护理、饮食护理、用药护理、满意程度等方面进行效果评价。结果实验组化学性腹泻发生率为26.67%,显著低于对照组的53.33%(P<0.05);实验组恶心呕吐发生率显著低于对照组(P<0.05)。干预组患者在心理护理、饮食护理、用药护理、满意程度等方面得分显著高于空白组(P<0.05)。结论加味黄芩汤中西医结合治疗和精细护理能减少开普拓所致严重腹泻的发生,提高患者生活质量。Objective To explore the effect of fine nursing on irinotecan-induced diarrhea by integrated treatment of traditional Chinese and western medicine. Methods Sixty patients were randomly divided into the experimental group and the control group. The experimental group was treated with huangqin decotion one day before treatment and control group was treated with conventional treatment. Incidence and degree of diarrhea and the incidence rate of adverse reactions such as bone marrow suppression,nausea and vomiting,anorexia were compared between two groups. The experimental group was randomly divided into intervention group and blank group. The blank group was given routine nursing and the intervention group was given fine nursing. Effect was evaluated from aspects of psychological nursing,diet nursing,and medication nursing and satisfaction degree. Results The incidence rate of chemical diarrhea was 26. 67% in the experimental group,which was significantly lower than 53. 33% in the control group（ P〈0. 05）. The incidence rate of nausea and vomiting in the experimental group was significantly lower than the control group（ P〈0. 05）. The intervention group were significantly higher than the blank group in scores of psychological nursing,diet nursing,medication nursing and satisfaction degree（ P〈0. 05）. Conclusion Integrated treatment of traditional Chinese and western medicine by huangqin decotion and fine nursing can reduce the incidence of irinotecan-induced diarrhea and improve quality of life of patients.
