摘 要:腹膜透析(PD)是终末期肾病(ESRD)患者有效的肾脏替代治疗方式。传统腹膜透析液(PDF)因高糖、低p H、高渗、葡萄糖降解产物(GDPs)等特点,生物相容性差,可导致腹膜结构和功能的破坏、炎症反应、代谢紊乱等不良反应,限制PD技术的长期应用。p H中性、低GDPs的PDF具有更好的生物相容性,但价格昂贵,限制其推广应用。因此,考虑到经济因素,应合理选择患者使用生物相容性好的PDF。本文就各类PDF的研究现状、不同PDF的选择做一综述。Peritoneal dialysis has been a well established renal replacement therapy for ESRD patients. However,conventional glucose-based peritoneal dialysis fluids with high glucose concentrations,low p H,high osmolality,glucose degradation products,could lead to activation of various inflammatory,metabolic disorders,and progressive damage of the peritoneal membrane structurally and functionally,and have been recognized as bio-incompatible,limiting long-term use of PD. PD solutions with a neutral p H and fewer glucose degradation products are more biocompatible,but the high price impede the popularizing process. Facing the reality of current financial constraints,an approach to individualizing the prescription of biocompatible PD solutions is sensible. In this paper,we review the current knowledge and dilemma of newer PD solutions,explore how we may individualize the prescription of biocompatible PD fluid.
