经会阴超声观察产后女性肛管直肠角初步研究

摘　　要：目的:运用经会阴超声对产后女性肛管直肠角进行观察,探讨角度变化的意义。方法:对自然分娩、剖宫产及正常未育女性各100例,运用腔内探头经会阴超声观察后盆腔,分别测量静息状态、缩肛状态及最大Valsalva动作时肛管直肠前角及后角的大小。结果:各组在缩肛状态及最大Valsalva动作时,肛直肠前角、后角均较静息状态时缩小。最大Valsalva动作时前角缩小率大于后角缩小率,差异具有统计学意义(P<0.05)。阴道分娩组、剖宫产组与未育组比较,最大Valsalva动作时前角缩小率较小,差异具有统计学意义(P<0.05)。阴道分娩组与剖宫产组比较,缩肛状态及最大Valsalva动作时肛直肠前角、后角的缩小率差异无统计学意义(P>0.05)。结论:经会阴超声分别观察肛直肠前角、后角,可间接反映相应耻骨阴道肌或耻骨直肠肌的肌力变化。Objective To investigate the meaning of observing angular variation on anorectal angle by transperineal ultrasonography.Methods 100 underwent spontaneous vaginal delivery and 100 underwent selective cesarean delivery（postpartum group） and 100 nulliparas（control group） were examined by transperineal ultrasonography.The anorectal anterior angle and posterior angle were separately measured at rest,on pelvic muscle contraction and during maximum Valsalva maneuver condition.Results On pelvic muscle contraction and during maximum Valsalva maneuver condition,the anorectal anterior angle and posterior angle in all groups were decreased than those at rest.The anterior angle reduced rate was bigger than posterior angle reduced rate during maximum Valsalva maneuver condition,there was signifi cant difference（P〈0.05）.During maximum Valsalva maneuver condition,the anterior angle reduced rate in postpartum groups was smaller than those in control group,there was signifi cant difference（P〈0.05）.Conclusions The anorectal anterior angle and posterior angle were separately measured by transperineal ultrasonography.It is potentially able to assess the contractility of pubovaginalis and puborectalis.
