干部保健人群高血压患者颈动脉内中膜厚度与骨量减少的相关性

摘　　要：目的探讨本地区干部保健人群原发性高血压患者动脉硬化与骨量减少的关系。方法选取中老年保健男性55例,其中原发性高血压患者30例,对照组25例。用双能X线骨密度仪检测左前臂中段、左股骨颈、腰椎（L1~4）总T值;彩色多普勒超声检测颈动脉内中膜厚度（IMT）。结果高血压组的下肢及腰椎骨量均低于对照组,颈动脉IMT均高于对照组（P〈0.05）。校正年龄、体质量指数后,颈动脉IMT与收缩压、低密度脂蛋白胆固醇呈正相关,与腰椎总T值呈负相关（P〈0.05）;按照骨量减少将原发性高血压患者30例分为两组,骨量异常组收缩压、舒张压、IMT值均高于骨量正常组（P〈0.05）。结论保健人群中高血压患者各部位骨量均有减低趋势,动脉硬化程度与骨量减少呈负相关,对于此类人群及早预防骨质疏松可能对延缓动脉硬化发展有一定作用。Objective To investigate the relationship between the carotid artery intima-media thickness（ IMT） and osteopenia in cadre health andmiddle-aged malepatients with essential hypertension in the local area. Methods 30 imiddle-aged malepatients with essential hypertension and 25 cases of the control group were included. Dual energy X-ray absorptiometry was used to detect the middle of left forearm,left femoral neck,lumbar spine（ L1 ~ 4） bone mass and colordoppler ultrasound was used to detect the carotid IMT. Results The hip and lumbar bone mass of hypertension group were significantly lower than those of control group,IMT of carotid artery was higher than that of control group（ P 0. 05）. After adjustment for age,body mass index,carotid artery IMT was positively related with systolic blood pressure and low density lipoprotein cholesterol,and negatively correlated with hip and lumbars＇ total T value（ P 0. 05）. According to the bone mass reduction,hypertension group was divided into abnormal bone mass group and bone mass reduction group. Systolic blood pressure,diastolic blood pressure,IMT values were higher in the reduction group than those of normal group（ P 0. 05）. Conclusions There is a tendency of reduction in the bone mass in cadre health patients with primary hypertension,and the degree of arterial stiffness is negatively correlated with the reduction of bone mass. Early prevention of osteoporosis might have some effects on the development of atherosclerosis.
