心脏专科医院与综合性医院室间隔修补术围手术期临床用血分析

摘　　要：目的分析不同类型医院室间隔修补术围手术期临床用血,为临床合理用血提供参考。方法对专科医院、大型综合性医院、地区综合性医院接受室间隔修补术的患者围手术期临床用血情况进行回顾性分析,分析红细胞、血浆和冷沉淀输注率与输注量的差异。结果专科医院室间隔修补术红细胞输注率(52.5%),输注量(1.00±0.99 U),显著低于大型综合性医院的输注率(95.3%)、输注量(3.25±1.77)U,亦显著低于地区综合性医院的医院的输注率(95.8%)、输注量(3.08±1.63),均有P<0.05。专科医院室间隔修补术的血浆输注率仅为1%,显著低于大型综合性医院(94.5%)和地区综合性医院(87.3%),P<0.01。大型综合性医院室间隔缺损术冷沉淀输注率为零,低于专科医院(1.5%)和地区综合性医院(2.5%),P<0.01。结论心脏专科医院在室间隔手术的为手术期临床用血控制良好,用血量显著低于综合性医院。Objective To analyze the actual blood usage during ventricular septal repair operation in different hospitals, providing useful reference for rational clinical use of blood. Methods The usage of blood components of patients during ven- tricular septal repair operation in specialized hospital, large general hospital, district general hospital were retrospectively an- alyzed. Infusion rate and quantity of red blood cell suspension, plasma and cryoprecipitate were obtained. A statistic analysis was conducted on the data. Results In specialized hospital, RBC transfusion rate （52. 5% ） and infusion volume （ 1.00 ± 0. 99 U） were significantly lower than those of large general hospital （95.3%）. The infusion volume （3.25 ± 1.77 U） was also significantly lower than that of district general hospital （95. 8% ） （3.08 ± 1.63） （P 〈0. 05）. In specialized hospital, the plasma transfusion rate was 1%, which was significantly lower than that of the large general hospitals （94. 5% ） and re- gional general hospitals （ 87.3% ）, （ P 〈 0. 01 ）. The cryoprecipitate infusion rate in large general hospital during ventricular septal defect surgery was zero, below that of specialized hospital （1.5%） and regional general hospitals （2. 5% ）, respec- tively （P 〈0. 01 ）. Conclusion Specialized hospital achieves well-controlled blood usage during ventricular septal repair operation, with a significant lower usage than in large and regional general hospitals.
