血浆制品有效成分含量分析

摘　　要：目的通过对本站制备的不同血浆制品中有效凝血因子的促凝活性,纤维蛋白原和血浆总蛋白含量的测定,为深入掌握血浆产品的质量、准确解答临床相关血浆输注问题提供基础数据支持,对指导临床合理用血并提高疗效提供帮助。方法随机抽取本站制备的新鲜冰冻血浆、普通冰冻血浆(去冷沉淀冰冻血浆31份,其他36份)、亚甲蓝病毒灭活冰冻血浆,共128份,分别用一期法检测FⅡ、FⅤ、FⅦ、FⅧ、FⅨ、FⅩ、FⅪ、FⅫ的促凝活性,凝血酶法检测纤维蛋白原和双缩脲法检测总蛋白含量,并进行统计学分析。结果血浆总蛋白各组间比较,差异无统计学意义(P>0.05)。其他检测项目组间比较:CRP与FP比较:FⅪ、FⅫ之间P值均>0.05,无统计学意义,其他项均有差异;CRP与MB-FP比较:Fib、FⅤ、FⅦ、FⅫ之间P值均>0.05,无统计学意义,其他项均有差异;CRP与FFP比较:Fib、FⅤ、FⅧ之间P<0.05有意义,其他项均无差异;FP与MB-FP比较:Fib、FⅡ、FⅦ、FⅪ之间P<0.05有统计学意义,其他项均无差异;FP与FFP比较:Fib、FⅡ、FⅫ之间P值均>0.05,无意义,其他项均有差异;MB-FP与FFP比较:FⅦ之间P>0.05,无统计学意义,其他项均有差异。结论不同血浆制品有效成分各不相同,临床上不能等同使用。经过灭活的MB-FP纤维蛋白原含量显著降低,与CRP水平相当,其他因子也有不同程度的降低;CRP因提取了冷沉淀与FP比较,纤维蛋白原含量显著降低,其他因子也各有差别,应作为一种成分与FP区别使用。Objective To provide basic data to support the clinical infusion of plasma, to guide the clinical reasonable use of blood, and to improve the curative effect in patients through determination of the procoagulant activity of coagulation factor, fibrinogen and the actual content of total protein in different plasma products in Qingdao Blood Center. Methods A total of 128 plasma products, such as fresh frozen plasma, frozen plasma （31 cryoprecipitate reduced plasma and 36 others） and methylene blue virus inactivated plasma were randomly collected. The procoagulant activity detection method was conducted to detect the activity of FII, FV, FVII, FVIII, FIX, FX, FXi, FXIi. Clauss method was used to detect Fibrinogen, and Biuret method was used to detect total protein content. Statistical analysis was conducted to evaluate the content of effective components in different kinds of plasma products. Re- suits From the comparison of plasma total proteins in each group, there was no significant difference （ P 〉 0. 05 ）. Other test items were compared between the groups： the comparison of FXI and FXII between CRP and FP showed no significant difference （P 〉0. 05）. Therewas no significant difference in the levels of Fib, FV, FVII and FXII between CRP and MB-FP （ P 〉 0. 05 ）. Significant difference was found in the activity of Fib, F V and FVIII between CRP and FFP （P 〈 0. 05）. While comparing the activity of Fib, FII, FVII and FXI between FP and MB-FP, significant difference was found （P〈0. 05）. The activity of Fib, F II and FXII between FP and FFP did not show significant difference （P 〉 0. 05）. The activity of FVR be- tween MB-FP and FFP also showed no significant difference （P 〉 0.05）. Conclusion The effective components in different kinds of plasma products are not same, which cannot be used equally.
