摘 要:目的观察光疗时间对臀部皮肤温度的影响,探讨预警评估干预在预防臀部皮肤损伤的效果。方法苏州市吴江区第一人民医院新生儿科2014年1月至4月收治住院的新生儿黄疸患儿100例,随机分为对照组和观察组各50例。两组新生儿均采用常规护理措施,治疗措施相同。对照组新生儿臀部皮肤予3M无痛保护膜保护,观察组新生儿光疗前臀部皮肤予3M无痛保护膜外喷,沿坐骨结节至肛周一圈外贴人工皮保护,人工皮外予3M9534HP贴膜保护,测量两组新生儿光疗前,光疗后2、4、6、8h患儿臀部皮肤温度,记录两组新生儿臀部皮肤损伤发生率。结果两组新生儿在光疗前、光疗后2、4h体温比较差异无统计学意义(P>0.05),而光疗6、8h对照组新生儿体温高于观察组,差异有统计学意义(P<0.05);两组新生儿光疗后2、4、6、8,各时间段新生儿体温均比光疗前显著升高,差异有统计学意义(P<0.01);观察组新生儿预警干预后臀部皮肤损伤发生率为2%(1/50),明显优于对照组16%(8/50),差异有统计学意义(P<0.05)。结论光疗期间造成患儿臀部皮肤温度上升,预警干预后臀部皮肤损伤发生率明显降低。Objective To study the influence of phototherapy time on the buttock skin temperature and to discuss the effect of early warning intervention in preventing the buttock skin damage.Methods Select 100 cases of jaundice in hospitalized neonates in Neonatal Department of the First People's Hospital of Wujiang from January to April 2014 and divide them randomly into conventional group and experimental group with 50 cases in each.The two groups were given routine nursing,and the same treatment.The buttock skin of the conventional group was protected by 3Mpainless protective membrane,while the experimental group was given 3Mpainless protective membrane protection before phototherapy and was post the artificial skin protection along the ischial tuberosity to anal Monday circle,with 3M9534 HP film coating protection outside the artificial skin.Measure the buttock skin temperature of the two groups before phototherapy and 2hours,4hours,6hours and 8hours after phototherapy.Record the incidence of buttock skin damage in the two groups.Results Body temperature before phototherapy,2hours and 4hours after phototherapy of the two groups had no significant difference(P〈0.05).However body temperature of 6hours and 8hours after phototherapy of the conventional group were higher than that of the experimental group,which had statistical significance(P〈0.05).Body temperature in each time period of 2hours,4hours,6hours and 8hours after phototherapy of the two groups were higher than that before phototherapy(P〈0.01).The incidence rate of the buttock skin damage after the early warning intervention of the experimental group was 2%(1/50),which was significantly better than the conventional group[16%(8/50)],the difference having statistical significance(P〈0.05).Conclusion Phototherapy causes the buttock skin temperature to rise and the incidence rate of buttock skin damage is reduced after the early warning intervention.
