摘　　要：目的研究2003-2014年海南省孕产妇死亡率与孕产期保健服务变化关系,为进一步降低孕产妇死亡率提供科学依据。方法利用海南省孕产妇死亡监测网络和妇幼卫生年报收集2003-2014年孕产妇死亡率及孕产期保健服务指标的数据,采用Cochran-Armitage趋势检验和相关分析研究孕产妇死亡率趋势及其与各项孕产期保健服务指标的关系。结果 2003-2014年海南省孕产妇死亡率平均年下降率为5.46%,呈现下降趋势;农村平均年下降率为7.64%,远高于城市的1.49%。孕产妇死亡率与各项孕产期保健服务指标呈负相关(P<0.05),前3位相关性指标为住院分娩率(γ=-0.94,P<0.001),产检率(γ=-0.92,P<0.001)和建卡率(γ=-0.89,P<0.001)。结论 2003-2014年海南省孕产妇死亡率下降明显,与孕产期保健服务密切相关。进一步加强孕产期保健服务质量,是实现千年发展目标5及"2011-2020年妇女儿童发展纲要规划"目标的有效途径。Objective To study the association of maternal heahhcare indicators with maternal mortality ratio （MMR） during 2003 to 2014 in Hainan province,and to provide scientific evidence for the further decline of MMR. Methods Data on MMR and maternal heahhcare indicators were collected by Maternal Mortality Surveillance network and Maternal and Child Health Annual Report in Hainan. Cochran - Armitage trend test and correlation analysis were used to analyze the association between MMR and maternal heahhcare indicators. Results The average annual reduction rate of MMT during 2003 to 2014 was 5.46% in Hainan （P for trend 〈0. 001 ） ,and the reduction in rural area was -7.64% ,which was much higher than 1.49% in urban area. Besides, MMR had negative correlations with maternal healthcare indictors ,and hospital delivery rate （ γ = - 0.94 ,P 〈 0. O01）,prenatal care rate （γ = -0.92,P〈0.001） and building card rate （γ = -0.89,P〈0.001） had stronger negative correlation. Conclusion MMR in Hainan province decreased significantly during 2003 to 2014 and had a strong correlation with maternal healthcare services. Improving the quality of maternal health care services is an effective approach to achieve the fifth Millennium Development Goals and ＂Outline Plan on Women and Child Development during 2011 -2020＂.
