摘　　要：目的根据四川凉山彝族自治州(凉山州)2013年吸毒人群、女性阳性者子女、阳性者配偶或性伴、孕产妇和临床病人艾滋病病毒(HIV)的感染状况,估算新发感染率。方法对凉山州2013年病例报告新发现的所有HIV-1感染者样本进行BED-CEIA HIV IgG捕获酶免疫测定法(BED HIV-1 incidence capture enzyme immunoassay,BED-CEIA)检测,并结合流行病学背景资料,估算多类监测人群的HIV新发感染率。结果在五类人群中,吸毒人群的HIV感染率和新发感染率均处于最高水平,分别为2.94%和1.65%;其次是女性阳性者子女,HIV感染率和新发感染率分别为2.23%和1.25%;阳性者配偶或性伴的HIV感染率为0.93%,新发感染率为0.26%;临床病人的HIV感染率为0.51%,新发感染率为0.31%;孕产妇的HIV感染率和新发感染率最低,分别为0.39%和0.25%。结论吸毒人群仍然是凉山州艾滋病防控的重点人群,同时也应关注艾滋病病毒的家庭内传播,尤其是母婴传播,在凉山州艾滋病已从高危人群蔓延到一般人群,艾滋病防控的力度和广度应继续加强。Objective To estimate human immunodeficiency virus type 1 （HIV - 1 ） incidence among drug users, children of H1V - positive mothers, people who have HIV - infected spouses/sexual partners, pregnant women and clinical patients in Liangshan Yi autonomous prefecture （Liangshan prefecture） in 2013. Methods BED capture enzyme immunoassay （ BED - CEIA） was adopted to detect all the HIV positive specimen reported in 2013 in Liangshan prefecture and the HIV incidence was estimated among the multiple populations with the epidemiological information. Results Among the multiple populations,drug users had the highest HIV prevalence （2. 94% ） and H1V incidence （ 1. 65% ）. Followed by children of HIV - positive mothers ,whose HIV prevalence was 2. 23% and HIV incidence was 1. 25%. The pregnant women had the lowest HIV prevalence （0. 39% ） and HIV incidence （0.25%）. The HIV incidence of people who had HIV infected spouses/sexual partners and clinical patients were 0. 26% and 0. 31% respectively. Conclusion Drug users are still the focus of HIV prevention and control in Liangshan prefecture. At the same time,we should be paid attention to HIV transmission in the family, especially the mother - to - child transmission. In Liangshan prefecture, the HIV had spread from high - risk groups to the general population, and the efforts should continually be strengthened and scaled up.
