我国乡镇卫生院适宜服务范围探讨

摘　　要：乡镇卫生院是农村三级卫生服务网络的核心与基础,但近年来乡镇卫生院的规模持续萎缩,国家多项基层改革举措成效难以体现。由于乡镇卫生院的服务范围不明确,造成农村居民就诊存在延误风险,农民服务利用趋高,导致乡镇卫生院服务进一步萎缩的恶性循环。当前乡镇卫生院需要一套明确的医疗服务范围作指导,乡镇卫生院范围的确定需要考虑具体服务的内容、关注需求还是效率、基于现状还是要求以及是否统一标准四项关键问题,建议从指导原则出发,结合实际情况,与服务指南相结合,同时明确管理服务范围,确定卫生院适宜的服务范围。Township hospitals were the foundation and core of the three-levels of rural health service network, but recently thescale of town hospitals was gradually shrinking and many national reform was difficult to achieve initial results at the primary level.Due to the unclear service scope in town hospitals, there were risks of delay in visiting doctors in rural areas. The utilization of ruralfarmer service was high, which resulted in vicious circle of further contraction for township hospital services. In current, the townshiphospitals needed a clear scope of medical services as a guide. The scope of medical services should consider the specific service con-tent, focus on the demand or efficiency, based on the current status or requirement, whether unify the standard. It suggested that thescope determination of appropriate services in town hospitals also needed the combination with guiding principles, the actual situation,service guide and the identification of management service scope.
