摘 要:目的比较CDFI、ADF及SMI检测乳腺病变血流的能力,探讨SMI在鉴别乳腺良恶性病变中的价值。方法纳入超声发现乳腺病灶患者198例(210个乳腺病灶),进行常规超声、CDFI、ADF及SMI检查,采用Adler分级,比较不同技术对同一病灶的血流检出能力。通过mSMI观察乳腺病变的血管形态分布特征,比较良恶性病变的不同表现。结果 mSMI对乳腺病灶的血流检出能力优于其他多普勒技术(P<0.001)。在mSMI模式下,良性病灶多表现为内部血管粗细均匀、走行自然伴或不伴周边环绕型血管;恶性病灶则常表现为内部血管粗大扭曲,周边可见放射状血管及穿支血管。良恶性病变的血管形态特征差异具有统计学意义(P<0.001)。结论 SMI可提高乳腺病变血流检出率,通过对乳腺病灶的血管形态分布特征进行观察,有助于良恶性病变的鉴别诊断。Objective To compare the ability of different Doppler techniques in blood flow detection of breast lesions,and to explore the potential of SMI in differential diagnosis.Methods The retrospective study included 198 patients with 210 breast lesions.All the lesions were examined by grayscale ultrasound,CDFI,ADF and SMI.Alder grading was applied to classify blood flow in breast lesions detected with different Doppler techniques.Morphologic and distribution features of blood flow were also observed with mSMI.Results mSMI was more sensitive to detect blood flow than other Doppler techniques(P〈0.001).On mSMI,blood vessels inside benign lesions were natural,straight with or without peripheral annular vessels.However,the blood vessels of malignant lesions manifested as large and twisted,with or without radial and penetrating vessels.Morphologic features were significantly different between benign and malignant lesions.Conclusions mSMI can increase blood flow detection of breast lesion,and depict morphologic and distribution features of breast lesions,which might be helpful for differential diagnosis.
