肾透明细胞癌的超声造影特点与肿瘤侵袭性的关系

摘　　要：目的探讨肾透明细胞癌(CCRCC)的超声造影增强模式及定量参数与肿瘤侵袭性的关系。方法回顾性分析75例肾透明细胞癌的超声造影表现并通过时间-强度曲线软件得到相关参数,根据病理结果,将肿瘤分为未侵犯组(41例)、侵犯组(34例),然后进行对比分析。结果 (1)CCRCC超声造影多表现为快进、快出或慢出、等或高增强,两组间比较差异无统计学意义(P>0.05);(2)CCRCC的不均匀增强多见于侵犯组(P<0.05),而CCRCC的假包膜征及长径≤3cm的肿块多见于未侵犯组(P<0.05);(3)侵犯组CCRCC的曲线尖度、曲线下面积及ΔPI均高于未侵犯组(P<0.05),穿透肾包膜的CCRCC在曲线尖度及ΔPI上均高于侵犯肾被膜者(P<0.05)。结论 CCRCC的超声造影特点与其侵袭性有一定的关系。Objective To explore the relationship among CEUS modalities and quantitative parameters of clear cell renal carcinoma（CCRCC）and tumor invasiveness.Methods CEUS appearance of 75 CCRCCs were retrospectively analyzed and got related parameters by time intensity curve software.According to the results of pathology,divided the tumors into non invasion group（41cases）and invasion group（34cases）,And then were analyzed.Results（1）the CEUS of CCRCC mostly showed quick in,quick out or slow out,iso/hyper-enhance,there was no significant difference between the two groups（P〈0.05）.（2）The inhomogeneous enhancement of CCRCC was more common in invasion group（P〈0.05）,while the psuedocapsule and the diameter≤3cm of CCRCC was more common in non invasion group（P〈0.05）.（3）Sharpness of time-intensity curve,the area under the curve andΔPI of CCRCC in invasion group were higher than those of the non invasion group（P〈0.05）,CCRCCs invaded and penetrated the capsule were higher than that invaded without penetrated the capsule on Sharpness of time-intensity curve andΔPI（P〈0.05）.Conclusions The CEUS characteristics of CCRCC have a certain relationship with tumor invasiveness.
