玻璃酸钠用于前交叉韧带重建术治疗胫骨平台骨折合并交叉韧带损伤的临床观察

摘　　要：目的:观察玻璃酸钠用于关节镜下前交叉韧带重建术治疗胫骨平台骨折合并交叉韧带损伤的临床疗效及安全性。方法:选取胫骨平台骨折合并交叉韧带损伤患者50例,按随机数字表法分为对照组和观察组,各25例。对照组患者行关节镜下前交叉韧带重建手术;观察组患者在对照组治疗的基础上给予关节腔内注射玻璃酸钠2.5 ml,1次/周,连续注射5周。观察两组患者临床疗效及治疗前后膝关节活动度,并比较治疗后视觉模拟(VAS)评分、骨代谢实验室检查指标及不良反应。结果:观察组患者总有效率为96.00%,显著高于对照组患者的80.00%,差异有统计学意义(P<0.05)。治疗后,两组患者膝关节活动度显著增大,且观察组明显高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者VAS评分及骨代谢实验室检查指标明显优于对照组,差异有统计学意义(P<0.05)。两组患者均未见明显不良反应发生。结论:透明质酸用于关节镜下前交叉韧带重建术治疗胫骨平台骨折合并交叉韧带损伤疗效显著,且安全性较好。OBJECTIVE：To observe clinical efficacy and safety of Sodium hyaluronate used in the reconstruction of anterior cruciate ligament under arthroscope for the treatment of tibial plateau fracture complicating with cruciate ligament injury. METHODS：50 patients with tibial plateau fracture complicating with cruciate ligament injury were collected and randomly divided into control group and observation group,with 25 cases in each group. Control group underwent anterior cruciate ligament under arthroscope;observation group was additionally given Sodium hyaluronate 2.5 ml,once a week,for consecutive 5 weeks,on the basis of control group. Clinical efficacy and range of knee joint motion were observed in 2 groups before and after treatment. VAS,bone metabolic markers and ADR were compared between 2 groups after treatment. RESULTS：Total effective rate of observation group was 96.00% in observation group,which was significantly higher than that of control group（80.00%）,with statistical significance（P〈0.05）. After treatment,range of knee joint motion improved significantly in 2 groups,and the observation group was higher than the control group,with statistical significance（P〈0.05）. After treatment,VAS and bone metabolic markers of observation group were significantly better than those of control group,with statistical significance（P〈0.05）. No obvious ADR was found in2 groups. CONCLUSIONS：Sodium hyaluronate used in the reconstruction of anterior cruciate ligament under arthroscope shows good therapeutic efficacy and safety for the treatment of tibial plateau fracture complicating with cruciate ligament injury.
